泮托拉唑治疗消化性溃疡合并上消化道出血的临床疗效观察

摘　　要：目的探讨泮托拉唑治疗消化性溃疡合并上消化道出血的临床疗效。方法入选对象为2010年7月～2015年6月确诊的消化性溃疡并发上消化道出血患者360例,将360例PUCH患者采取随机数字表法随机分为对照组和观察组,每组各180例,两组患者均予补充血容量、维持水电解质平衡、保护胃肠黏膜等对症治疗。治疗组加用泮托拉唑40 mg加入0.9%生理盐水250 m L中静滴,每日2次,连用3～10 d;对照组予立止血1KU加入0.9%生理盐水100 m L中静滴,每日2次。比较两组治疗后的止血效果及止血时间、住院时间。结果观察组治疗后总有效率93.3%,显著高于对照组治疗后总有效率70.0%,观察组治疗后的疗效优于对照组,差异有统计学意义（P〈0.05）。观察组患者治疗的止血时间显著短于对照组,观察组患者的住院时间也短于对照组,差异有统计学意义（P〈0.05）。结论泮托拉唑治疗消化性溃疡合并上消化道出血疗效确切,止血效果好,值得推广和应用。Objective To investigate the effect of pantoprazole in treatment of peptic ulcer complicated with upper gastrointestinal bleeding. Methods Selected 360 patients with digestive ulcer complicated digestive tract hemorrhage from July 2010 to June 2015, were randomly divided into control group and observation group according to the random number table method, each with 180 cases, two groups of patients were given supplement blood volume, maintaining water and electrolyte balance, protected the gastrointestinal mucosa and symptomatic treatment. Treatment group was treated with 40 mg pantoprazole added 0.9% physiological saline 250 m L intravenous injection, 2 times a day, for 3-10 days;the control group received reptilase 1 KU adding 0.9% saline 100 m L by intravenous drip, 2 times a day. Blood routine, liver and kidney function and ECG were examined before and after treatment in all patients. The hemostatic effect and the time of stopping bleeding and hospital stay time were compared between two groups. Results The total effective rate of the observation group was 93.3%, significantly higher than that of the control group（70.0%） after treatment,the difference was significant（P〈0.05）. The time of stopping bleeding in observation group was significantly shorter than that in control group, and the length of hospital stay in observation group was shorter than that in control group, the difference was significant（P〈0.05）. Conclusion Pantoprazole in the treatment of digestive ulcer complicated with upper digestive tract hemorrhage has curative effect, good hemostatic effect, worthy of promotion and application.
