前部玻璃体切除治疗恶性青光眼的疗效观察

摘　　要：目的观察前部玻璃体切除术对恶性青光眼治疗的临床效果。方法对2013年3月到2015年2月在河北省眼科医院就诊的31例(42只眼)恶性青光眼进行回顾性分析,包括手术前后视力、眼压、前房深度及并发症等情况。结果 42只眼术前眼压平均(32.15±4.17)mm Hg,术后眼压平均/(14.07±2.15)mm Hg,经统计学计算术前术后眼压差异具有统计学意义(t=P<0.01)。前房深度由术前(0.956±0.37)mm加深到(1.83±0.41)mm,经统计学计算术前术后前房深度差异具有统计学意义(t=P<0.05)。结论前部玻璃体切除治疗恶性青光眼可以破坏玻璃体水囊,沟通前后房,使前房加深,阻止房水逆流,有效的降低眼压,是治疗恶性青光眼的一种有效地手术方法。Objective To investigate the effect of anterior vitrectomy to the treatment of malignant glaucoma.Method 42 cases of malignant glaucoma patients,who were in-patient in Hebei Eye Hospital between March 2013 and February 2015,were involved in this study.The visual acuity,intraocular pressure,anterior chamber depth and complication were evaluated Pre-and post-operatively.Results The average preoperative IOP of the 42 eyes was 32.15±4.17 mm Hg,and 14.07 ± 2.15 mm Hg postoperatively.The change of IOP has statistically significant difference（t=P〈0.01）.The anterior chamber depth deepened from 0.956 ± 0.37 mm preoperatively to 1.83 ± 0.41 mm postoperatively.It has statistically significant difference（t=P〈0.05）.Conclusion Anterior vitrectomy is an effective surgical method for the treatment of malignant glaucoma,which can damage vitreous bladders,collude the anterior chamber and posterior chamber,deepened anterior chamber,prevent reverse flow of aqueous humor,effectively reduce intraocular pressure.
