胃低分化腺癌患者中P53、KI-67、EGFR与HER-2的表达水平及其与预后的相关性

摘　　要：目的:探究分析P53、KI-67、EGFR与HER-2在胃低分化腺癌患者血清中的表达水平及其预后的相关性。方法:采用免疫组化技术研究55例人正常胃黏膜组织以及60例胃低分化腺癌组织标本中Her-2,EGFR,Ki-67,P53蛋白的表达特点及其与淋巴结转移和预后的关系,并通过图像分析对免疫组化标记结果进行定量分析,从而探讨四者与胃低分化腺癌发生发展的关系。结果:胃低分化腺癌患者的过表达率分别为P53(41%)、KI-67(18.5%)、EGFR(17.9%)与HER-2(55.2%),胃低分化腺癌患者的性别、年龄、组织分级、肿瘤分型等临床病理因素与KI-67、EGFR与HER-2表达无关(P<0.05),P53表达与胃低分化腺癌患者的组织学分级相关。胃低分化腺癌转移及淋巴结转移对患者的无病生存时间及总生存时间具有一定影响。结论:胃低分化腺癌患者血清P53、KI-67、EGFR及HER-2水平的检测在患者预后判断及评估中具有显著的指导价值,为临床上分子靶向治疗的研究提供了借鉴资料。Objective: To explore the analysis of P53,EGFR and its ehrs,KI-67-2 expression in stomach poorly differentiated adenocarcinoma patients serum level and its prognostic relevance.Methods:Using immunohistochemical technique research of 55 cases of normal gastric mucosa tissues and its ehrs in 60 cases of stomach poorly differentiated adenocarcinoma tissue samples-2,EGFR,Ki-67,the expression of P53 protein and its relationship with lymph node metastasis and prognosis,and through image analysis carries on the quantitative analysis results of immunohistochemical markers,which discuss four relationship with the occurrence and development of gastric poorly differentiated adenocarcinoma.Results:The stomach poorly differentiated adenocarcinoma patients over expression rate of P53,respectively(41%),KI-67(18.5%),EGFR(17.9%) and its ehrs-2(55.2%),stomach poorly differentiated adenocarcinoma patients' age,gender,organization classification,clinical pathologic factors such as tumor classification and KI-67,EGFR has nothing to do with its ehrs-2 expression(P<0.05),the expression of P53 is associated with histologic classification of stomach poorly differentiated adenocarcinoma patients.Stomach poorly differentiated adenocarcinoma metastasis and lymph node metastasis in patients with disease-free survival and overall survival time has a certain impact. Conclusion:The stomach poorly differentiated adenocarcinoma patients serum P53,EGFR and its ehrs,KI-67-2 levels of detection in the patient's prognosis judgement and assessment has significant guiding value,provides the reference to the research of molecular targeted therapy clinical data.
