655例卵巢肿瘤术中快速冷冻病理检查的准确性分析

摘　　要：目的通过分析卵巢肿瘤术中快速病理检查与常规病理检查结果的符合率来评估快速病理检查在不同卵巢肿瘤中的诊断准确性。方法回顾性收集655例卵巢肿瘤术中快速冷冻病理检查结果以及术后常规病理检查结果,分析恶性、交界性、良性卵巢肿瘤快速冷冻病理检查和常规病理检查的符合率以及诊断的灵敏度、特异度、阳性和阴性预测值。结果 655例卵巢肿瘤快速冷冻病理检查与常规病理检查总符合率为94.7%,在恶性、交界性以及良性肿瘤中确诊符合率分别为91%、82.5%和97%。其灵敏率、特异率、阳性预测值、阴性预测值在良性肿瘤中分别为97%、89.2%、95.5%和92.6%;在交界性肿瘤中分别为82.5%、98.5%、78.6%、98.9%;在恶性肿瘤中分别为91%、100%、100%和97.3%。结论术中快速冷冻病理检查能够快速并且准确的对卵巢肿瘤进行分类及定性诊断,对卵巢肿瘤的切除范围与术式的选择提供了可靠的依据。Purpose This retrospective study was undertaken to evaluate the accuracy of introperative frozen section in the diagnosis of ovarian tumors. Methods Restrospective study of 655 ovarian frozen section results was examined to determine the accuracy of frozen section diagnosis. The diagnosis obtained by intraoperative frozen section was compared with the permanent section and the overall accuracy, sensitivity, specificity, positive predictive and negative predictive values of frozen section were studied. Results The overall accuracy to determine the status of malignancy was 94. 66%. The accuracy rates for malignant, benign, and borderline ovarian tumors were 90. 97%, 82. 50% and 96. 96% respectively. The sensitivity, specificity, positive predictive and negative predictive values for the benign tumors were 96. 96%, 89. 23%, 95. 50% and 92. 55% respectively, for borderline tumors were 82. 50%, 98.54%, 78. 57% and 98. 86% respectively, and for malignant tumors were 90. 97%, 100%, 100% and 97. 28% respectively. Conclusion This retrospective study concluded that frozen section is a reliable diagnostic procedure for the histopathological diagnosis of ovarian tumors.
