摘 要:目的探讨乳腺癌再发甲状腺癌与单发甲状腺癌ER表达的情况。方法回顾性分析21例乳腺癌再发甲状腺癌和21例单发甲状腺癌的临床特征、病理组织学类型,SP法检测ER并进行统计学分析。结果 21例乳腺癌再发甲状腺癌均为女性,平均年龄48.2岁;21例单发甲状腺癌均为女性,平均年龄45.5岁。前者中19例为乳腺癌非特殊型浸润性导管癌,1例为黏液癌,1例为浸润性小叶癌;而再发甲状腺癌均为甲状腺乳头状癌。后者均为甲状腺乳头状癌。21例乳腺癌再发甲状腺癌患者中,乳腺癌16例ER(+),5例(-);甲状腺癌4例ER(+),17例(-),且4例ER(+)的甲状腺癌来自于ER(+)的乳腺癌;而21例单发甲状腺癌ER均为(-)。ER在乳腺癌再发甲状腺癌患者中的乳腺癌和甲状腺癌中表达差异显著(P<0.01),ER在乳腺癌再发甲状腺癌与单发甲状腺癌中表达差异不显著(P>0.01)。结论乳腺癌与甲状腺癌均具激素依赖性,两者需相互排除,以便早期发现、早期治疗。同时对激素依赖性甲状腺癌患者可尝试进行内分泌治疗。Objective To investigate the expression of ER in breast cancer patient occurred thyroid cancer and simple thyroid cancer. Methods Analyze retrospectively the clinic features and histological types and use IHC to test the expression of ER of 21 breast cancer occurred thyroid cancer patients and 21 simple thyroid cancer patients admitted from Jan. 2010 to Aug. 2014 in the third affiliated hospital of kunming medical university. And we do statistical analysis for the IHC results. Results The 21 breast cancer occurred thyroid cancer patients were female,the average age was 48.2 years old; and 21 simple thyroid cancer patients also were female, the average age was 45.5 years old. In 21 breast cancer occurred thyroid cancer patients,the breast cancer histological types are invasive ductal cancer(19) ,muciuous carcinoma( 1 ), invasive lobular carcinoma( 1 ) ;the thyroid cancer histological type is papillary thyroid carcinoma. The histological type of 21 simple thyroid cancer patients is also papillary thyroid carcinoma. Among 21 breast cancer occurred thyroid cancer patients, 16 breast cancer patients ER( + ) ,5ER( - ) ;4 thyroid cancer ER( + ), 17 thyroid cancer ER( - ). 4 thyroid cancer ER( + ) patients came from breast cancer ER( + ) patients. 21 simple thyroid cancer ER( - ). The difference between the expression of ER in the tumors of thyroid and breast has statistical significance ( P 〈 0. 01 ), The difference between the expression of ER in the tumors of breast cancer occurred thyroid cancer and simple thyroid cancer has no statistical significance ( P 〉 0. 01 ). Conclusion Breast cancer and thyroid cancer are hormone dependent. For patients of breast cancer, thyroid cancer should be exclude and vice versa. It is helpful for early detection and early treatment. The hormonal dependent thyroid cancer patient may get endocrine therapy. It maybe accumulate data for evidence based guideline development.
