大肠癌复发转移相关因素的Logistic分析

摘　　要：目的探讨大肠癌复发、转移患者的相关因素,为大肠癌患者预后评估提供参考。方法对2008年1月-2009年1月皖南医学院第一附属医院收治的75例大肠癌患者进行5年的随访,观察患者复发、转移的情况。对患者的年龄、性别、Dukes分期、术后组织病理分化类型、术前血清肿瘤标记物(CA-125、AFP、CA-199、CEA)、术后肿瘤组织免疫组化标记物(Ki-67、Her-2、APC、nm23、CEA)等进行Logistic回归分析。结果单因素分析结果显示,Dukes分期,分化类型,血清肿瘤标记物CEA、CA-199,术后肿瘤组织免疫组化标记物Ki-67、Her-2、nm23、CEA与大肠癌患者复发、转移具有相关性(P<0.05)。Logistic多因素回归分析结果显示,Dukes分期,血清肿瘤标记物CEA,术后肿瘤组织免疫组化标记物Her-2、CEA与大肠癌患者复发具有相关性(P<0.05);Dukes分期、血清肿瘤标记物CEA、术后肿瘤组织免疫组化标记物Her-2与大肠癌患者转移具有相关性(P<0.05)。结论大肠癌的Dukes分期,血清肿瘤标记物CEA,肿瘤组织免疫组化标记物Her-2、CEA可能是大肠癌患者预后判断有价值的指标。Objective To explore the related factors of colorectal cancer, and provide the basis for evaluating the prognosis of patients with colorectal cancer. Methods Seventy-five patients with colorectal cancer who admitted in the First Affiliated Hospital of Wannan Medical College from Jan. 2008 to Jan. 2009 whether recurrence and metastasis or not were observed by 5 years of follow-up. Their ages, genders, Dukes stages, types of tissue differentiation, tumor markers （CA-125, AFP, CA-199, CEA）, and the results of immunohistochemical markers （Ki-67, Her-2, APC, nm23, CEA） were analyzed by Logistic regression analysis. Results Univariate analysis showed that Dukes staging, differentiation, serum tumor markers CEA, CA-199, postoperative tumor tissue immunohistochemical markers Ki-67, Her-2, nm23, CEA were related with recurrence and metastasis of colorectal cancer （ P 〈 0.05 ）. Logistic multivariate regression analysis showed that Dukes staging, serum tumor markers CEA, postoperative tumor tissue immunohistochemi- cal markers Her-2, CEA were related with recurrence of colorectal cancer （P 〈 0.05） ; Dukes staging, serum tumor markers CEA, postoperative tumor tissue immunohistochemical markers Her-2 were related with metastasis of colorectal cancer （P 〈 0.05 ）. Conclusion Dukes stages, serumtumor marker CEA, postoperative tumor tissue immunohisto- chemical markers Her-2 and CEA can be used as valuable indicators for evaluating the prognosis of patients with eolorec- ta｜ cancer.
