摘 要:目的探讨Toll样受体(Toll-like receptor,TLR)2、4、5及髓样分化因子88(My D88)、核因子κB(NF-κB)表达在H.pylori相关的胃炎与胃溃疡(gastric ulcer,GU)病变免疫反应中的作用。方法选取青岛市常住居民中慢性胃炎198例、GU 82例、对照组90名。根据快速尿素试验及Giemsa确定各组H.pylori感染率;并利用实时定量聚合酶链反应(Real-time PCR)技术分析各组的各个基因mRNA的表达。结果 GU组、胃炎组、对照组三组之间的H.pylori感染率差异有统计学意义(χ2=13.95,P<0.01)。H.pylori阳性组中TLR2、TLR4、My D88及NF-κB的mRNA表达明显高于H.pylori阴性组(P<0.001);H.pylori一致时,GU组TLR2、TLR4、My D88及NF-κB mRNA表达量明显高于胃炎组及对照组(P<0.01)。结论 H.pylori相关的胃炎与GU的发病可能与H.pylori通过TLR途径作用于机体免疫系统有关。Objective To investigate the role of Toll-like receptor 2 (TLR2), TLR4, TLR5 and the signaling pathways in the gastric mucosa diseases induced by observing the patients with H. pylori infection. Methods Biopsy specimens were obtained from the gastric mucosa of 198 patients with gastritis, 82 patients with gastric ulcer (GU) and 90 healthy persons by the gastroscopy. Rapid urea test and Giemsa staining were performed to observe the infection rates of H. pylori. The expressions of TLR2, TLR4, TLR5, NF-κB and MyD88 mRNAs were detected by Real-time PCR. Resuits The infection rates of H. pylori in GU group, gastritis group and control group were 74.4% , 61.6% , 46.7% , there was significant difference (X2 = 13.95, P 〈 0. 01 ). The expression levels of TLR2, TLR4, NF-κB and MyD88 mRNA were significantly higher in the H. pylori positive group than the H. pylori negative group (P 〈 0. 001 ). Smilarly, no matter in the H. pylori positive group or in the H. pylori negative group, the expressions of TLR2, TLR4, NF-κB and MyD88 mRNAs in GU group were higher than gastritis group and control group (P 〈 0.01 ). Conclusion H. pylori may induce the begin and development of the gastric mucosal diseases by upregulating the expression and the signal transduction of TLRs.
