摘  要:目的通过调查由1起聚餐引起的以发热、腹痛、腹泻为主要症状的聚集性疫情,查找传染源和传播方式,为今后类似疫情提供防控依据。方法制定统一病例定义,开展病例主动搜索,对就餐人员就餐情况开展回顾性队列研究,采集标本进行实验室检测。结果共发生44例病例,疑似病例12例,临床诊断病例26例,实验室诊断病例6例,其中43例为参加宴席病例,罹患率86.0%(43/50),1名餐馆从业人员发病。从病例标本(粪便或肛拭子)分离到6株韦太夫雷登沙门菌,从鸡场污水排放口中分离到1株韦太夫雷登沙门菌,7株沙门菌PFGE鉴定结果显示为同一型别。对5月27日就餐的51例就餐者开展回顾性队列研究。结果显示5月27日中午的凉菜为危险因素。结论该起疫情为宴席中使用的鸡肉受韦太夫雷登沙门菌污染所致,污染来源可能来自制作的凉拌鸡的鸡肉。Objective By investigating and analyzing symptoms of fever, abdominal pain, diarrhea after a rural dinner, to find the contaminate source and mode of transmission, provide scientific evidence for developing and control measures. Methods Unified diagnostic criteria of case was designed. Initiative searching cases and retro- spective cohort investigation was conducted. The samples were collected from the cases and environment for la- boratory tests. Results In total 44 cases were reported, including 12 suspected cases, 26 clinical diagnosis cases and 6 confirmed cases. There were 43 cases attended the same banquet, the attack rate was 86.0% (43/50), one employee case, six strains of salmonella weltevreden were detected in stool samples or rectal swabs from 10 cases. One strain of salmonella weltevreden was detected in water sample from chicken house pollutants discharge port, with the same PFGE. Retrospective cohort investigation was conducted in all diners who attended the ban- quet at noon on May 27. Cold dishes at noon on May 27 were risk factors. Conclusion The outbreak of salmonel- la is caused by salmonella weltevreden, the suspected source of pollution may come from the chicken salad.
