宫腔镜与腹腔镜切除术对子宫壁间肌瘤患者围手术期临床指标及妊娠结局的影响

摘　　要：目的:探讨宫腔镜与腹腔镜子宫肌瘤切除术对子宫壁间肌瘤患者围手术期临床指标及妊娠结局的影响。方法:选取近期收治的150例子宫壁间肌瘤患者,采用随机数字表法分为A组(n=75)与B组(n=75),分别采用腹腔镜、宫腔镜手术治疗。对比两组患者围手术期临床指标、术后肌层愈合率、术后复发率、首次妊娠距手术时间、术后妊娠率及新生儿出生体重等。结果:B组患者手术时间、术中出血量及血红蛋白降低量均优于A组,差异有统计学意义(P<0.05);B组患者术后3个月、6个月肌层愈合率均高于A组,差异有统计学意义(P<0.05);两组患者术后复发率、术后妊娠率、新生儿出生体重差异无统计学意义(P>0.05);B组患者首次妊娠距手术时间短于A组,差异有统计学意义(P<0.05)。结论:宫腔镜手术治疗子宫壁间肌瘤可有效缩短手术时间,减少术中创伤,加快术后康复进程,更适于有生育要求的患者。Objective ： To investigate the influence of hysteroscopic and laparoscopic myomectomy on clinical indicators in perioperative period and pregnancy outcome of patients with uterine intramural myoma. Methods：One hundred and fifty patients with uterine intramural myoma in recent years were chosen and divided by random digital table method into group A （ n = 75, laparoscopy myomectomy） and group B （n = 75, hysteroseopy myomectomy）. The clinical indicators in perioperative period, postoperative muscle healing rate, recurrence rate, time to the first pregnancy from operation, postoperative pregnancy rate and neonatal birth weight of both groups were compared. Results： The operation time, blood loss and amount of hemoglobin decreased of group B were significantly better than those of group A （ P 〈 0.05 ）. The postoperative muscle healing rate in 3 and 6 months after operation of group B was significantly better than group A （ P 〈 0.05 ）. There was no significant difference in recurrence rate, postoperative pregnancy rate and neonatal birth weight between 2 groups （ P 〉 0.05 ）. The time to the first pregnancy from operation of group B was significantly shorter than group A （ P 〈 0.05 ）. Conclusions ： Compared with laparoscopy myomectomy, hysteroscopy myomectomy in the treatment of uterine intramural myoma can efficiently shorten the operation time,reduce the degree of intraoperative trauma and accelerate postoperative recovery process, is more suitable for patients with the requirements of birth.
