D－二聚体与血浆纤维蛋白原比值测定在颅内静脉系统血栓形成诊断中的价值

摘　　要：目的：检测颅内静脉系统血栓形成（CVST）患者血浆纤维蛋白原（Fg）、D－二聚体（D－D）的水平，探讨D－D／Fg（ D／F）比值在CVST诊断中的价值。方法2013年3月—2014年10月收治的33例CVST患者，所有患者均行Fg测定，其中19例患者同时测定D－D并计算D／F值。门诊健康体检者23例作为对照组。结果CVST组Fg水平与对照组比较差异无统计学意义（P＞0．05）；CVST组D／F比值及D－D水平与对照组比较差异有统计学意义（P＜0．01），其中D－D诊断CVST的灵敏度（Se）为94．7％，特异度（Sp）为95．7％，D／F比值诊断CVST的Se为94．7％，Sp为100％。结论血浆D／F比值及D－D水平可以作为筛查诊断CVST快捷的实验室指标，其中D／F比值诊断CVST更有特异性。Objective To determine the levels of plasma fibrinogen （ Fg） and D-dimer （ D-D） in patients who were diagnosed with cerebral venous sinus thrombosis （ CVST） , in order to assess the application of D-D/Fg （ D/F） ra-tio in the diagnosis of CVST.Methods Clinical Data were collected from 33 CVST patients admitted into the Affiliated Hospital of Xuzhou Medical College from March 2013 to October 2014.All patients underwent Fg testing, of which 19 patients were also examined for D-D and D-D/Fg ratio.Meanwhile, 23 health subjects were included as the control. Results The CVST group and the control group showed no significant difference in Fg level （P〉0.05）.Compared with the control, the CVST group produced remarkably different D/F ratio and D-D levels （P〈0.01）.If D-D was adopted to diagnose CVST, the sensitivity was 94.7%and the specificity was 95.7%.If D/F ratio was used to diagnose CVST, the sensitivity was 94.7%and the specificity was 100%.Conclusions Plasma D/F ratio and DD level can be adopted as laboratory parameters to effectively screen and diagnose CVST, in which D/F ratio produced better specificity.
