细胞周期素A表达与胃癌预后关系的研究

摘　　要：目的探讨细胞周期素A(Cyclin A)在胃癌组织中的表达及其与肿瘤预后的关系。方法回顾性分析125例行胃癌根治术患者的临床病理资料,采用免疫组织化学方法检测组织中Cyclin A的表达情况,对比Cyclin A阳性表达与阴性表达患者临床病理资料的差异,并对患者的预后进行单因素、多因素分析。结果Cyclin A阳性表达者78例,阴性表达者47例。与阴性组比较,阳性组病理分化类型较差、肿瘤直径较大、浸润深度深、淋巴结转移多、TNM分期晚,差别均具有统计学意义(P<0.05)。阳性组患者术后5年生存率为30.8%,明显低于阴性组的72.3%。单因素分析显示,肿瘤大小、Lauren分型、分化类型、浸润深度、淋巴结转移和TNM分期和Cyclin A表达与患者预后相关。多因素Cox回归分析显示,浸润深度、淋巴结转移和TNM分期是患者预后的独立影响因素。结论 Cyclin A阳性表达的胃癌患者,肿瘤恶性度较高,病期较晚,预后较差;Cyclin A可以作为预后判断的辅助指标。Objective To investigate the expression of Cyclin A in gastric cancer and to assess the correlation of Cyclin A to prognosis of patients gastric cancer. Methods 125 cases of gastric cancer patients at Fujian Medical University Union Hospital were analyzed retrospectively. We examined the ex- pression of Cyclin A by immunohistochemistry. The survival rate was calculated by the Kaplan-Meier method, the Log-rank test and Cox＇s proportional hazards regression model using SPSS 16.0, Results There were 78 patients （62. 4%） with the positive expression level of Cyclin A in gastric cancer. Compared with the negative group, cases in the positive group have poor pathologic types, larger tumor, and more advanced cancer. The 5-year survival rate in patients with positive and negative Cyclin A expression was 30. 8% and 72. 3%, respectively, with the difference being statistically significant （P〈0.05）. Univariate survival analysis showed that the tumor size, Lauren types, pathologic types, depth of tumor invasion, lymph node metastasis, TNM stage, and Cyclin A were associated with the prognosis. Multivariate analysis demonstrated that depth of tumor invasion, lymph node metastasis, and TNM stage were independent risk factors for the prognosis （P〈0.05）. Conclusion Patients with positive expression of Cyclin A are prone to poor pathologic changes andadvaneed cancer, leading to worse outcomes. Cyclin A may be identified as a biomarker for predicting the prognosis of gastric cancer.
