摘　　要：目的评价一蒂多岛的分叶股前外侧皮瓣在头颈部恶性肿瘤术后复杂缺损修复中的应用价值。方法 2009年8月-2015年6月,应用以旋股外侧动脉降支为蒂部的一蒂多岛的分叶股前外侧皮瓣一期修复头颈部恶性肿瘤术后复杂缺损44例,分析分叶股前外侧皮瓣在复杂缺损修复中的优势及手术难点。结果 42例患者皮瓣成活,术后缺损重建区的形态及其功能均令人满意;供区无明显后遗症。结论股前外侧皮瓣因其组织结构的多样性和丰富的旋股外侧血管穿支,在复杂的头颈部软组织缺损重建中较其他单一的皮瓣具有较明显的优势,是一种可靠的、多能的修复皮瓣。Objective To evaluate the free polyfoliate perforator flaps of anterolateral thigh for reconstruction in complicated oral and maxillofacial defects. Methods From August 2009 to June 2015, the ramified musculocutaneous flaps pedicled with the descending branch of lateral circumflex femoral artery were used to repair oral and maxillofacial defects after radical tumor resection in 44 eases. The advantages and disadvantages of the free polyfoliate perforator flaps were analyzed. Results There were 42 out of 44 flaps survived completely. The appearance and function of the operation region were satisfactory in postoperative follow-up. No obvious complications were observed at the donor sites. Conclusion Because of the diversity of tissues and rich perforator vessels, anterolateral thigh flap is reliable and ver- satile in repairing complicated oral and maxillofacial defects. This type of flap shows obvious advantages as compared with other types of flaps.
