神经上皮肿瘤扩散加权成像b值优化选择的初步研究

摘  要:目的使用1.5T超导MR扫描仪选取不同扩散敏感系数(b值)对神经上皮肿瘤病例行颅脑扩散加权成像(DWI)检查,遴选最佳b值,为获取最佳图像质量。方法随机选择42例经手术病理证实的脑神经上皮肿瘤患者行颅脑DWI检查,扩散敏感梯度因子b值分别采用0、700、900、1100、1300、1500s/mm2,分别对应为A、B、C、D、E、F六组。每组均测量肿瘤病灶表观扩散系数(ADC)值,病变处的信号强度(s病变),相应层面脑白质信号强度(s脑白质),并计算出对应SNR、CNR,通过对比六组ADC,SNR、CNR,对比不同b值对成像效果的影响。结果 D组的SNR、CNR、ADC分别高于A、B、C、E、F组,C组的SNR、CNR、ADC分别高于A、B、E、F组(P<0.05),E组的SNR、CNR、ADC分别高于A、B、F组,E组的SNR、CNR、ADC分别高于A、B、F组,B组的SNR、CNR、ADC分别高于A、F组,F组的SNR、CNR、ADC分别高于A组,差异有统计学意义(P均<0.05)。结论应用扩散加权成像,当b值为1100s/mm2时,对于神经上皮肿瘤具有最佳的成像效果,值得临床推广。Objective To determine appropriate b-value in diffusion weighted imaging of neuroepithelial tumor with 1. 5T MR, in order to get the best image quality. Methods 42 patients with neuroepithelial tumor received DWI scaning of brain using a single shot EPI-SE pulse sequence with different b values(0s/mm^2, 700s/mm^2, 900s/mm^2, 1100s/mm^2, 1300s/mm^2,1500 s/mm^2), the corresponding A, C, B, E, D, F six groups were respectively, and the ADC values of the lesion were measured. The image quality of each group of DWI and ADC were analyzed and compared, compared the effects of different b values on the imaging effect. Results The images of DWI and ADC of 42 patients were all captured clearly, and ADC values of lesions were measured too. As b values increasing from 1100s/mm^2 to 1500s/mm^2, SNR,CNR of brain white matter and lesion decreased gradually. When b value was 1100s/mm^2, the ADC value was highest, and then decreased step by step. When b value was1100s/mm^2,the reference rang of ADC value was stable too. Conclusion When b value is 1100s/mm^2, the image quality of DWI and ADC map of neuroepithelial tumor is the best one, and it is worthy of clinical promotion.
