摘 要:目的研究阿尔茨海默病(Alzheimer’s disease,AD)和帕金森病(Parkinson’s disease,PD)患者血清代谢物的变化。方法收集正常人和临床确诊的AD和PD患者的血清,采集各组血清样品的NMR1H谱,运用主成分分析法(principalcomponentanalysis,PCA)和正交最小偏二乘判别法(orthogonal partial leastsquares-discriminant analysis,OPLS-DA)分析。结果 PCA和OPLS-DA分析显示正常人、AD患者和PD患者的血清代谢特征存在明显差异。与正常人相比,AD患者血清中异亮氨酸和乳酸含量明显升高(P<0.05),脂质、谷氨酰胺、肌酸和葡萄糖含量明显降低(P<0.05);PD患者血清中乳酸和谷氨酰胺含量升高(P<0.05,P<0.01),葡萄糖的含量降低(P<0.01);与AD患者相比,PD患者血清中脂质、乳酸和谷氨酰胺含量明显升高(P<0.05,P<0.01),而异亮氨酸、丙氨酸和葡萄糖的含量明显降低(P<0.05,P<0.01)。结论 AD患者与PD患者在血清代谢水平上具有明显差异,具有明显变化的代谢物(谷氨酰胺、丙氨酸、异亮氨酸、脂质、乳酸和葡萄糖)有可能作为潜在血清代谢标记物,为临床区分AD和PD提供参考。Objective To determine the serum metabolic changes in the patients with Alzheimer’s disease (AD) and Parkinson’s disease (PD). Methods Serum samples of normal human, AD and PD patients diagnosed clinically were collected, and then NMR 1H spectra were performed and analyzed by principal component analysis (PCA) and orthogonal partial least squares-discriminant analysis (OPLS-DA). Results PCA and OPLS-DA analyses showed that there were great differences in serum metabonomics among normal human, AD and PD patients. Compared with normal people, AD patients had higher levels of isoleucine and lactate (P〈0.05), and lower levels of lipid, glutamine, creatine and glucose (P〈0.05), while the levels of lactate and glutamine were increased (P〈0.05, P〈0.01), and that of glucose was decreased in PD patients (P〈0.01). The serum levels of lipid, lactate and glutamine were significantly higher (P〈0.05,P〈0.01), while those of isoleucine, alanine and glucose were lower in the PD patients than the AD patients (P〈0.05, P〈0.01). Conclusion Distinct differences exist in the serum metabolic profiles between the AD and PD patients, and these distinct metabolites may serve as potential biomarkers to distinguish AD from PD in clinical practise.
