摘 要:目的探讨肝细胞癌表皮生长因子受体(EGFR)不同表达水平者的超声造影特征及量化情况。方法回顾分析78例经手术病理证实为肝细胞癌患者的超声造影检查资料,其中EGFR表达阴性患者9例(EGFR表达阴性组),EGFR表达阳性患者69例(EGFR表达阳性组),阳性组中弱阳性者27例,明显阳性者42例,使用超声造影软件分析病灶的造影剂到达时间(AT)、达峰时间(TTP)、峰值强度(PI)、流出时间(WT)和增强曲线下面积(AUC),结果进行比较。结果 EGFR表达阳性组肝细胞癌病灶超声造影时"快进快出"的特征更为明显。EGFR表达阳性组AT、TTP和WT均早于EGFR表达阴性组,PI高于EGFR表达阴性组,AUC小于EGFR表达阴性组(均P<0.05)。EGFR表达弱阳性者和明显阳性者的AT、TTP、PI、WT及AUC比较,差异均无统计学意义。结论通过分析EGFR与超声造影量化参数之间的关系,可应用超声造影量化参数预测肝细胞癌生长和进度。Objective To explore contrast-enhanced ultrasound (CEUS) parameters of hepatocellular carcinoma (HCC) with different epidermal growth factor receptor(EGFR) expression. Methods The CEUS data of 78 patients with HCC confirmed by surgery pathology were collected, they were divided into EGFR positive group (69 cases, including 27 cases of weak positive expression and 42 cases of significantly positive expression ) and EGFR negative group (9 cases ) according to the expression of EGFR. The ultrasonic imaging software was used to calculate the CEUS parameters, including arrival time (AT), time to peak (TIP),peak intensity (PI),washout time (WT) and area under the curve (AUC),the difference between the two groups was compared by statistics test. Results The imaging features of "quick in-quick out" was more apparent in EGFR positive group than that in the EGFR negative group. The parameters including AT,TIP and WT in EGFR positive group were all earlier than those in EGFR negative group. The PI in EGFR positive group was higher than that in EGFR negative group. The AUC in EGFR positive group was less than that in EGFR negative group (P〈O.05).But compared the parameters including AT, TIP, PI, WT and AUC between weak positive group and significantly positive group, there was no statistical significance. Conclusion By analyzing the relationship between contrast-enhanced ultrasonic parameters on hepatocellular carcinoma with different EGFR expression ,the prognosis of HCC could be predicted.
