中、低位直肠癌应用腹腔镜直肠全系膜切除术治疗的临床效果探讨

摘　　要：目的探讨中、低位直肠癌应用腹腔镜直肠全系膜切除术治疗的临床疗效。方法整群选择该院2012年1月—2015年6月进行中、低位直肠癌治疗的患者72例,随机分为对照组与观察组,每组36例。观察组予以腹腔镜下行直肠全系膜切除术,对照组行开腹直肠癌全系膜切除术。术后观察两组患者的临床疗效以及术后并发症发生情况。结果观察组患者的手术时间长于对照组,但手术中出血量、术后排气时间、下床时间以及住院时间观察组均明显少于对照组,且差异具有统计学意义(P<0.05)。观察组切口感染、性功能障碍、肠梗阻和尿潴留的发生率明显低于对照组,且差异具有统计学意义(P<0.05),吻合口瘘两组数据差异无统计学意义(P>0.05)。结论中、低危直肠癌应用腹腔镜行直肠全系膜切除术疗效确切,患者痛苦少,并发症少,具有良好的治疗效果和安全性。Objective To observe the clinical effect of laparoscopic total mesorectal excision on middle and low rectal can-cer. Methods 72 cases with middle and low rectal cancer admitted in our hospital from January 1st, 2012 to June 30th, 2015 were selected and randomly divided into the observation group and the control group, each group of 36 cases. The ob-servation group were given laparoscopic total mesorectal excision and the control group were given open total mesorectal excision. The clinical efficacy and incidence of postoperative complications of the two groups were observed. Results The operation time of the observation group was longer while the intraoperative blood loss, postoperative exhaust time, ambula-tion time and hospital stay of the observation group were significantly less than that of the control group, respectively, the differences were significant statistically significant （P〈0.05）. The incidence of incision infection, sexual dysfunction, intesti-nal obstruction and urinary retention of the observation group was significantly lower than that of the control group, respec-tively, the differences were statistically significant（P〈0.05）. There was no statistical difference in the incidence of anastomot-ic fistula between the two groups （P〉0.05）. Conclusion Laparoscopic total mesorectal excision has exact effect on middle and low rectal cancer with less pain, rapider recovery and better therapeutic effect and higher safety.
