胃癌原发灶与癌周淋巴结中LYVE-1、VEGF-C及VEGFR-3表达的研究

摘  要:目的探讨淋巴管透明质酸受体-1(LYVE-1)、血管内皮生长因子C(VEGF-C)及血管内皮细胞生长因子受体-3(VEGFR-3),在胃癌原发灶和癌周淋巴结中表达差异及其与微淋巴管密度(LVD)的关系。方法采用基于免疫组织化学染色法的组织微阵列(TMA)技术检测125例胃癌组织、600枚癌周淋巴结、96例癌旁正常组织中LYVE-1、VEGF-C及VEGFR-3的表达水平,并对Podoplanin标记的微淋巴管密度(LVD)计数。结果 LYVE-1、VEGF-C及VEGFR-3在胃癌原发灶与癌周淋巴结中的阳性表达率均高于癌旁正常组织中的表达率(P<0.05);在胃癌原发灶与癌周淋巴结中三因子阳性表达率,分别比较差异无统计学意义(P>0.05),在非转移性淋巴结中阳性表达率高达48.0%、51.0%及47.0%。LVD表达量在胃癌原发灶为(4.22±1.09)个(瘤周)、2.98±0.81(瘤内)及癌周淋巴结为(4.14±1.56)个,均高于癌旁正常组织中的1.82±0.63(P<0.05),但在胃癌原发灶与癌周淋巴结中相比差异无统计学意义(P>0.05),并与LYVE-1、VEGF-C及VEGFR-3阳性表达率呈正相关性(P<0.05)。结论通过检测胃癌组织、癌周淋巴结中LYVE-1、VEGF-C和VEGFR-3表达水平,可以有效预测和评估胃癌患者潜在的淋巴结微转移情况。Objective To discuss the differential expression of lymphatic vessel hyaluronan receptor- 1( LYVE- 1),Vascular endothelial growth factor C( VEGF- C) and vascular endothelial growth factor receptor- 3( VEGFR- 3) in primary sites and peritumoral lymph nodes of gastric carcinoma and the relationship with lymphatic vessel density. Methods The expression of LYVE- 1,VEGF- C and VEGFR- 3 in 125 cases of gastric carcinoma primary sites,96 cases of adjacent normal tissue and 600 pieces of lymph nodes were detected by tissue microarray which based on Immunohistochemistry testing. Results The rate of LYVE- 1,VEGF- C and VEGFR- 3 positive expression in primary sites and lymph nodes were significantly higher than adjacent normal tissue( P 〈0. 05),there was no significant difference among the expression of the three protein in primary sites and lymph nodes( P 〉0. 05). The rate of LYVE- 1,VEGF- C and VEGFR- 3 positive expression was respectively 48. 0%,51. 0% and 47. 0% in lymph nodes without metastasis. The contents of LVD in primary sites was intratumoral( 2. 98 ± 0. 81),4. 22 ± 1. 09 around the tumor and 4. 14 ± 1. 56 in lymph nodes,which was higher than that in the adjacent normal tissues( P 〈0. 05). There were no significant difference between primary and peritumoral lymph nodes( P 〉0. 05). Furthermore,expressions of LYVE- 1,VEGF- C and VEGFR- 3 had a positive correlation with LVD( P 〈0. 05). Conclusion It can effectively predict and evaluate potential lymph node micrometastasis in patients with gastric cancer by testing the expression levels of LYVE- 1,VEGF- C and VEGFR- 3 in gastric carcinoma and peritumoral lymph nodes.
