摘  要:目的通过数字减影血管造影(DSA)检测脑动静脉畸形血管构筑特征,探讨脑动静脉畸形合并出血与静脉引流血管构筑特点之间的相关性。方法回顾性分析经DSA检查确诊的145例脑动静脉畸形出血患者,综合DSA、CT和MRI影像学检查,采用Logistic回顾性分析,对畸形血管团的位置、引流静脉数目,有无伴发静脉狭窄、静脉扩张及静脉回流等静脉构筑因素与畸形血管团破裂出血的关系进行分析。结果 Logistic回归分析显示,脑动静脉畸形患者当存在单支静脉引流(OR为1.88,95%CI为1.26~3.38;P〈0.05)及深部引流(OR为3.22,95%CI为1.67~6.86;P〈0.05)时易出血。结论脑动静脉畸形的静脉特征分析可能有助于确定其预后,从而确定病变最适合的治疗方法。Objective To investigate the association between venous characteristics and intracranial arteriovenous malformation hemorrhage. Methods Retrospectively analyzed 145 patients with Ccerebral arteriovenous malformation hemorrhage patients. DSA,CT and MR imaging studies were evaluated to classify lesion side,drainage pattern,venous stenosis,number of draining veins,venous ectasia and venous reflux. Logistic regression analyses were performed to identify the association of these angiographic features with intracranial hemorrhage. Results Exclusively deep drainage( OR = 3. 22; 95% CI,1. 67- 6. 86; P 〈0. 05) and a single draining vein( OR = 1. 88;95% CI,1. 26- 3. 38; P 〈0. 05) were associated with AVMs hemorrhage,whereas venous ectasia( OR = 0. 48; 95% CI,0. 37 ~ 0. 79;P 〈0. 05) was inversely associated with hemorrhage. Conclusion Analysis of venous characteristics of intracranial AVMs may help to determine their prognosis and the most appropriate treatment.
