β阻滞剂联合碳酸酐酶抑制剂对新生血管性青光眼术后眼压及细胞因子的影响

摘　　要：目的：研究β阻滞剂联合碳酸酐酶抑制剂对新生血管性青光眼术后眼压及细胞因子的影响。方法：选择2012-05/2014-10期间接受手术治疗的血管新生性青光眼患者74例80眼为研究对象,根据入院时间先后顺序随机分为观察组37例41眼和对照组37例39眼,观察组接受β阻滞剂联合碳酸酐酶抑制剂治疗,对照组患者接受β阻滞剂单药治疗,比较两组患者眼压水平、血清细胞因子含量及治疗效果。结果：（1）临床疗效：观察组有效率93%明显高于对照组74%（χ~2=4.931,P〈0.05）;（2）眼压水平：治疗后3mo,观察组平均眼压、眼压峰值、眼压波动值明显低于对照组（t=13.422,11.555,8.129,P〈0.05）;（3）细胞因子：治疗后7d,观察组血清与房水中白细胞介素-6（IL-6）、血管内皮生长因子（VEGF）含量明显低于对照组,色素上皮衍生因子（PEDF）含量明显高于对照组（t=11.762,7.608,5.275,3.780,7.450,2.625,P〈0.05）。结论：β阻滞剂联合碳酸酐酶抑制剂有助于降低血管新生性青光眼患者的术后眼压水平,调节细胞因子的合成,提高治疗效果。AIM：To study the effect of β-blockers combined with carbonic anhydrase inhibitors on postoperative intraocular pressure（IOP） control of neovascular glaucoma.·METHODS：Seventy- four patients（80 eyes） with neovascular glaucoma received surgical treatment in our hospital from May 2012 to October 2014 were randomly divided into observation group and control group.Observation group received β-blockers combined with carbonic anhydrase inhibitors treatment.Control group received monotherapy of β-blocker.Then IOP,cytokine levels,curative effect were compared between the two groups.·RESULTS：Curative effect：the efficacy rate of observation group（93%） was significantly higher than that of control group（74%,χ~2 = 4.931,P0.05）;IOP level：at 3mo after treatments,average IOP,IOP peak,IOP fluctuation of observation group were significantly lower than those of the control group（t = 13.422,11.555,8.129,P0.05）;Cytokines：at 7d after treatments,serum and aqueous humor vascular endothelial growth factor（VEGF） and interleukin-6（IL-6） of observation group content were significantly lower than those of the control group,pigment epithelium derived factor（PEDF） was significantly higher than that of the control group（t =11.762,7.608,5.275,3.780,7.450,2.625,P0.05）.·CONCLUSION：The therapy of β-blockers combined with carbonic anhyd rase inhibitors can help to reduce the level of postoperative IOP in neovascular glaucoma patients,regulate the synthesis of cytokines,and then improve therapy effect.
