摘  要:目的探讨剥夺睡眠脑电图(EEG)对于小儿复杂部分性癫痫的诊断价值及其影响因素。方法对96例临床确诊为复杂部分性癫痫的儿童进行剥夺睡眠EEG检查和常规脑电图检查,比较两种诊断方法的阳性率。对比剥夺睡眠EEG检查阳性组患儿与阴性组患儿的有关临床资料,并对相关影响诊断的临床指标作Logistic回归分析。结果剥夺睡眠EEG检查阳性率为73.96%(71/96),显著高于常规脑电图检查的47.92%(46/96),差异具有统计学意义(P<0.01)。剥夺睡眠EEG检查阳性组的发作频率及有发展诱因的比例明显低于阴性组,而发作持续时间及有缺氧窒息史的比例则要高于阴性组(P<0.05)。Logistic回归分析显示,发作诱因、发作频率及发作持续时间是影响剥夺睡眠EEG阳性率的关键因素(P<0.05)。结论剥夺睡眠脑电图是小儿复杂部分性癫痫的有效诊断手段,发作频率、发作持续时间及发作诱因是影响其诊断率的关键因素。Objective To investigate the diagnostic value of sleep-deprived EEG in pediatric complex partial seizures(CPS) and its influence factors. Methods 96 CPS children were implemented sleep-deprived EEG and routine EEG examination to compare the positive rate of CPS. The relevant clinical data were compared between positive children and negative children diagnosed by sleep-deprived EEG,and the relevant factors were analyzed by multivariate Logistic regression analysis. Results The CPS positive rate of sleep-deprived EEG was 73.96%(71/96), significantly higher than 47.92%(46/96) of routine EEG, with statistical difference(P〈0.01). Compared with sleep-deprived EEG negative group, the episodes frequency and rate of precipitating factors of positive group decreased significantly, and the episodes duration and rate of hypoxic asphyxia increased significantly(P〈0.05). The results of Logistic regression analysis showed that attack inducement, episodes frequency, episodes duration are the key factors of sleep-deprived EEG(P〈0.05). Conclusions Sleep-deprived EEG is a practical method for the diagnosis of pediatric CPS. The episodes frequency, episodes duration and attack inducement are the key factors of the diagnostic rate of sleep-deprived EEG.
