腹腔镜低位直肠癌根治术与传统开腹手术的临床应用比较分析

摘　　要：目的比较分析腹腔镜低位直肠癌根治术与传统开腹手术的临床应用,探讨腔镜手术的优势。方法回顾性分析我科2009年8月~2011年8月共50例腹腔镜低位直肠癌根治术与同期68例传统开腹直肠癌根治术患者的临床病理及解剖资料,同时比较术后住院天数、手术总时间、术中总出血量、清扫的淋巴结个数、手术费用、排气时间、术后出现并发症及生存率情况。结果临床资料显示在性别、年龄、BMI、病理分型和浸润深度及术后清扫淋巴结总数方面,两组差异无统计学意义。两组手术出血量分别为（46.73±21.34）ml和（133.5±21.23）ml,术后排气时间为（3.09±0.46）d和（5.01±1.00）d,术后住院天数为（6.84±1.03）d和（11.72±1.19）d,差异有统计学意义（P〈0.05）。术后并发症、生存时间、复发和转移率比较均无统计学差异。结论腹腔镜低位直肠癌根治术是安全、可靠、微创的手术方式,相比开腹手术具有明显优势,可广泛在不同级别医院开展。Objective To analyze the clinical application of laparoscopic and open radical resection of lower rectal can- cer, to explore the advantages of laparoscopic surgery. Methods From Aug 2009 to Aug 2011, 50 patients with lower rectal cancer underwent laparoscopic radical resection, while 68 cases received open resection. The clinical pathological data and anatomy were collected and compared, including the operation, postoperative hospital stay, amount of blood loss, number of lymph node dissection, exhaust time, operation costs, postoperative complications and survival rate. Results There were no statistical significance between the two groups in gender, age, body mass index, pathological type, depth of invasion lymph node dissection. The bleeding volume of the two group were （46.73±21.34）m1 and （133.5±21.23）ml respectively, the postoperative exhaust time were （3.09±0.46）d and （5.01±1.00）d, the length of hospital stay were （6.84±1.03）d and （11.72±1.19）d, and the difference was significant （P〈0.05）. The postoperative complications, survival, recurrence and metastasis rate were compared between the two groups, the differences were not statistically significant. Conclusion Laparoscopic radical resection is a fea- sible, safe and minimally invasive technique for lower rectal cancer, is superior to open, suitable for different level hospitals.
