小肠出血47例诊治分析

摘　　要：目的探讨小肠出血的发病原因、临床诊断和外科治疗。方法回顾性总结我院1988年3月至2013年12月期间经手术治疗的47例小肠出血性疾病的临床资料。结果经手术和病理证实小肠肿瘤出血31例,小肠憩室出血6例,克罗恩病4例,血管畸形4例,出血性肠炎2例。术后早期发生肠梗阻2例,切口感染2例,肺部感染1例,全部治愈。结论小肠出血的常见原因有小肠肿瘤、憩室、炎症和血管畸形,术前联合几种检查可提高该病的诊断率。外科手术是小肠出血的有效治疗方法。Objective To investigate the causes,diagnosis and treatment of small intestinal bleeding. Methods The clinical data of 47 patients with small intestinal bleeding confirmed by surgery from March 1988 to December 2013 were analyzed,retrospectively. Re-sults Small intestinal tumor bleeding（31 cases）,small intestinal diverticulum bleeding（6 cases）,Crohn′s disease（4 cases）,vascular abnormalities（4 cases）,hemorrhagic colitis（2 cases）,were confirmed by surgery. Intestinal obstruction（2 cases）,incision infection（2 cases）,pulmonary infection（1 cases）occurred primarily after surgery,and all of those cases were cured by operation. Conclusion The incidence of intestinal bleeding was caused by many factors,such as the lack of specific diagnostic criteria,small intestine cancer, diverticulitis,inflammation and vascular malformations. Preoperative joint centralized examination can improve the diagnosis of the dis-ease. Surgery is the most effective treatment.
