原发性胃恶性淋巴瘤与幽门螺杆菌感染的相关性初探

摘　　要：目的探讨原发性胃恶性淋巴瘤（PGL）与幽门螺杆菌（Hp）的相关性。方法选取2002年3月至2015年1月就诊的原发性胃恶性淋巴瘤41例，采用Envision二步法进行免疫组织化学检查，采用免疫组化染色的方法检测幽门螺杆菌。以40例同期慢性萎缩性胃炎患者为对照组。结果41例原发性胃恶性淋巴瘤有MATJT淋巴瘤19例，DLBCL21例，Burkitt淋巴瘤1例：MALT淋巴瘤抗Hp的阳性率（36．8％）显著高于非MALT淋巴瘤（14-3％），差异有统计学意义（P〈0．05）；慢性萎缩性胃炎抗Hp阳性率（75．0％）显著高于胃MAI圩淋巴瘤、胃非MALT淋巴瘤的抗Hp阳性率，差异有统计学意义（P〈0．01）。结论幽门螺杆菌感染与胃MALT淋巴瘤密切相关，但其幽门螺杆菌感染率低于慢性萎缩性胃炎。Objective To investigate the relationship between Helicobacter pylori （Hp） infection and primary gastric lymphoma （PGL）.Methods 41 patients with PGL in our hospital from March 2002 to January 2015 were selected,and studied with im- munohistochemical examination.Hp were stained with anti-Hp.40 cases of chronic atrophic gastritis patients in our hospital were selected as the control group.Results There were MALT lymphoma 19 cases,DLBCL 21 cases,and one case of Burkitt lymphoma;The frequency of anti-Hp positivity is 36.8% in MALT lymphoma and 14.3% in non-MALT lymphoma,with a statistical significance（P〈 0.05）;The frequency of anti-Hp positivity in chronic atrophic gastritis is 75.0% and it is higher than that in gastric MALT lymphoma and gastric non-MALT lymphoma （P 〈 0.01）.Conclusion There is close correlation between Hp infection and MALT lymphoma,but the infection rate of Hp in. primary gastric lymphoma is lower than that in chronic atrophic gastritis.
