多发伤患者死亡率与6h乳酸清除率相关性研究

摘　　要：目的探计多发伤患者死亡率与6h血乳酸清除率的相关性。方法根据出重症监护病房（ICU）时的救治结果将88例多发伤患者分为死亡组和存活组；所有患者入ICU后立即采动脉血气分析查血乳酸浓度，治疗6h后再次采动脉血复查血乳酸浓度并计算6h动脉血乳酸清除率，对两组的乳酸浓度绝对值及6h乳酸清除率进行分析比较。结果入ICU当时时动脉血乳酸浓度：死亡组：（6．77±2．66）mmol／L，存活组：（5．05±2，74）mmol/L；6h乳酸清除率：死亡组为（16．33±7．47）％，存活组为（39．14±11．33）％（P〈0．01）。结论多发伤患者死亡率与6小时乳酸清除率之间呈明显负相关，在判断疗效以及评估预后方面有较大的临床价值，且操作简单，简便快捷．值得推广。Objective This study aimed to assess the severity and prognosis of multiple trauma patients by early blood lactate clearance rate.Methods Arterial blood samples were collected from 88 multiple trauma patients in the ICU of Second Affiliated Hospital of Dalian Medical University from period from january 2012 to Norvember 2015.These patients were divided into death group and survival group,and 6h after admission to Critical Care Medicine for blood gas analysis, the lactate clearance rate was calculated. The correlation of lactate clearance rate with the clinical outcome of the patients was analyzed.Results The blood lactate： survival group were 5.05±2.74, fatal group were 6.77±2.66,6h lactate clearance rates in survival group 39.14±11.33%, and fatal group were 16.33±7.47%, （P 〈 O.01）.Conclusion The 6h lactate clearance rates is a good index to assessment of the severity and the prognosis for multiple trauma patients better than blood lactate.
