摘  要:目的研究氨基胍(AG)对家兔心脏骤停后心肺复苏(CPR)中心功能的影响。方法于2015年4月—2015年7月取40只家兔用胸外膜电击法建立心肺复苏模型,平分为对照组、肾上腺素组、AG、AG加肾上腺素组。监测平均动脉压(MAP)、右房压、左室最大变化速率(Dp/dt),计算冠脉灌注压(CPP)。结果胸外按压3 min,肾上腺组、氨基胍及氨基胍联合肾上腺素组提高MAP、Dp/dt、CPP,明显强于空白对照组(P<0.01)。胸外按压5 min时,AG组及AG联合肾上腺素组提高MAP、Dp/dt、CPP,强于肾上腺组(P<0.05)。自主循环恢复后至复苏后240 min,AG组及AG联合肾上腺素组亦强于肾上腺素组(P<0.01),肾上腺素组与空白对照组间差异无统计学意义(P>0.05)。结论氨基胍能减轻家兔心肺复苏后早期心功能不全的发生。Objective To study the effect of aminoguanidine (AG) on the cardiac function in rabbits with sudden cardiac ar- rest after cardiopulmonary resuscitation (CPR). Methods 40 rabbits were made as the models of cardiopulmonary resuscita- tion by thoracic electric shock method from April 2015 to July 2015 and equally divided into the control group, adrenaline group, AG group, AG combined with adrenaline group. Mean arterial pressure (MAP), right atrial pressure, left ventrieular maximal change rate (Dp/dt) were measured and coronary perfusion pressure(CPP) was calculated. Results After 3 minutes of chest compression, the MAP, Dp/dt and CPP increased more obviously in the adrenaline group, AG group, AG combined with adrenaline group than the control group (P〈0.01). After 5 minutes of chest compression, the MAP, Dp/dt, and CPP in- creased more significantly in the AG group, AG combined with adrenaline group than in the adrenaline group (P〈0.05). From the recovery of spontaneous circulation to 240 minutes after CPR, the MAP, Dp/dt and CPP also increased more sig- nificantly in the AG group, AG combined with adrenaline group than in the adrenaline group (P〈0.01), while there was no sta- tistically significant difference in those indicators between the adrenaline group and the control group (P〉0.05). Conclusion The occurrence of early cardiac dysfunction after cardiopulmonary resuscitation in rabbits was reduced.
