内镜下粘膜切除术治疗大肠息肉的临床探讨

摘　　要：目的探讨内镜下粘膜切除术治疗大肠息肉的临床疗效。方法整群选取2011年6月—2014年12月该院收治的120例大肠息肉患者的207枚〉0.5 cm息肉,进行内镜下粘膜切除术,标本送病理检查,术后随访。结果息肉切除后病理检查结果炎性息肉2枚,增生性息肉12枚,管状腺瘤170枚,绒毛状管状腺瘤16枚,绒毛状腺瘤7枚,其中伴有中重度不典型增生53枚,局部癌变3枚,经病理确认癌变仅限制于粘膜层未侵及粘膜下肌层,故未进行肿瘤根治术。术后无严重出血和穿孔等并发症。有78例患者6~12个月后进行了肠镜复查,未发现切除部位的息肉复发,3例息肉癌变的患者随访6~24个月未见复发和转移。结论采用内镜下粘膜切除术对大肠广基和亚蒂息肉进行治疗,是非常安全和有效的。Objective To discuss the clinical curative effect of endoscopic mucosal resection in treatment of colorectal polyps..nethods 207 polyps〉0.5cm from 120 cases of patients with colorectal polyps treated in our hospital from June 2011 to December 2014 were selected and were given endoscopic mucosal resection, the specimens were sent for pathological examination, and the patients were followed up after operation. Results After polyp resection, pathological examination results show 2 polyps were inflammatory polyp, 12 polypswere hyperplastic polyp, 170 polyps were tubular adenoma,16 polyps were villous tubular adenoma, 7 polyps were villous adenoma, among them, 53 polyps were with moderate and severe dysplasia, 3 polyps were field cancerization, pathology confirmed that cancerization was only restricted to the mucous membrane layer not invaded and mucous membrane so it was not given tumor radical operation, there was no complications such as severe bleeding and perforate after operation, 78 cases of patients were given enteroscopy reexamination after 6-12 months, recurrence of polyps in the resection position was not found, recurrence and transform were not found after 6-24 months of follow-up in 3 cases of patients with polyp cancerization. Conclusion Endoscopic mucosal resection in treatment of large intestine and Iati polyps is very safe and effective.
