乳腺粘液癌的超声诊断与误诊防范

摘　　要：目的探讨乳腺粘液癌的彩色多普勒声像图特征。方法回顾性分析该院2012年10月—2014年11月经手术病理证实的21例乳腺粘液癌患者归入观察组,并与同期确诊的30例浸润性导管癌归入对照组,将两组的声像图特征进行比较,主要观察肿块的形态、大小、边界、内部回声、钙化情况、有无后方回声减弱或增强现象、血供情况、阻力指数、腋窝淋巴结肿大与否等。结果观察组患者经超声诊断后发现肿瘤最大直径范围0.63~4.01 cm,对照组患者肿瘤最大直径范围0.78~5.02 cm,观察组乳腺粘液癌发生于左侧乳腺者占60.00%,高于右侧乳腺的40.00%,但差异无统计学意义（P〉0.05）。超声检查对乳腺粘液癌的诊断符合率为71.43%。在典型的乳腺粘液癌中,提示肿块边界模糊、形态无规则性、不均与的内部回声,提示后方回声增强,无丰富血供、少数患者存在钙化现象。结论乳腺粘液癌的声像图有一定的特征性,检查者提高对其声像图认识,有助于提高其诊断率,降低误诊及漏诊。Objective To discuss the color Doppler sonographic features. Methods 21 cases of patients with mucinous car- cinoma of breast confirmed by surgical pathology in our hospital from October 2012 to December 2014 were retrospectively analyzed and were regarded as the observation group, 30 cases of patients with infitrating ductal carcinoma confirmed at the same period were regarded as the control group, the sonographic features of the two groups were compared, shape, size, border, internal echo, calcification, rear echo weakened or enhanced phenomenon existing or not, blood supply, drag index and axillary lymph nodes enlargement or not were mainly observed. Results Tumor maximum diameter range was 0.63-4.01 cm in the observation group and 0.78-5.02 em in the control group by ultrasonic diagnosis, in the observation group, the ratio of mucinous carcinoma of breast occuring in the left breast was higher than that in the right breast （60.00% vs 40.00%）, however, the difference was not statistically significant（P〉0.05）, the diagnostic accordance rate of ultrasonic examination for mucinous carcinoma of breast was 71.43%, the typical mucinous carcinoma of breast reminded that mass boundary was fuzzy , there was no irregular shape, the internal echo was uneven, rear echo was enhanced, there was no rich blood supply and a small number of patients was with calcification phenomenon. Conclusion The sonography of mucinous carcinoma of breast has a certain mark, and it contributes to improving the diagnostic rate and decreasing the misdiagnosis and missed diagnosis for examiner to improve the recognition of sonography.
