摘　　要：目的初步探讨孔缘牵张术联合自体血清治疗玻璃体切除术后未闭合的特发性黄斑裂孔手术疗效。方法对2012年6月至2013年6月在滨州沪滨眼科医院诊治的4例经23G玻璃体切除联合内界膜剥除治疗后黄斑裂孔未愈合的特发性黄斑裂孔患者，采用孔缘牵张术联合自体血清治疗，通过眼底检查和光学相干断层扫描观察术后黄斑裂孔愈合情况。结果4例病人4周复查黄斑裂孔全部解剖复位愈合。术后随访半年期间，患者病情稳定，手术无并发症出现。结论孔缘牵张术联合自体血清是治疗初次手术后特发性黄斑裂孔未愈合的有效方法。但目前仍需要大样本、长期的研究以评价此种手术方式的安全性和有效性。Objective To observe the effectiveness of Hole edge distraction therapy with autologous serum for unclosed idiopathic macular holes. Methods Four consecutive cases of idiopathic macular hole were included in the study. All the four eyes had undergone 23G vitrectomy and internal limiting membrane peeling previously but the macular holes were not closed. Hole edge distraction therapy with autologous serum was performed for all four cases. Fundus examination and optical coherence tomography were used to observe whether the macular holes were closed after the operation. Results Macular holes were closed in all four cases, and the situation was stable during the follow-up period of 6 months. No complication was recorded. Conclusions Hole edge distraction therapy with autologous serum technique is helpful in treating unclosed idiopathic macular holes after initial surgery. Long-term studies in larger sample are needed to further evaluate the efficacy and safety of this technique.
