摘 要:目的 利用影像学分析技术探讨胶质母细胞瘤同源性磷酸酶-张力蛋白基因(PTEN)表达的脑区分布.方法 采集首都医科大学附属北京天坛医院神经外科201 1年5月至2013年3月间收治的143例胶质母细胞瘤患者的磁共振影像,分别进行肿瘤病变范围标记,并配准到MNI坐标系标准脑图谱上.基于体素的定量影像学分析,计算每个体素PTEN表达水平的平均值,并通过伪彩色阶显示在标准脑图谱上.结果 143例胶质母细胞瘤患者中,有75例患者纳入到PTEN高表达组,68例患者纳入到PTEN低表达组,两组之间年龄、性别等临床资料差异无统计学意义(P>0.05).肿瘤累及全脑分布图显示:本组患者胶质母细胞瘤多发生于两侧室管膜下区.胶质母细胞瘤PTEN表达水平脑图谱显示:PTEN高表达相关脑区主要位于右侧额叶;而PTEN低表达相关脑区主要位于右侧颞叶后部.结论 定量影像学分析技术可以用来确定胶质母细胞瘤中PTEN表达和解剖位置之间的相关性,为胶质母细胞瘤发生过程的遗传学改变过程提供了新的证据.Objective To investigate the brain region distribution of phosphatase and tensin homology (PTEN) expression using imaging analysis techniques.Methods The magnetic resonance images of 143 patients with glioblastoma admitted to the Department of Neurosurgery,Tiantan Hospital,Capital Medical University from May 2011 to March 2013 were collected.The tumor lesion range marker of each patient was conducted respectively and was registered to the standard brain atlas on the MNI coordinate system.The voxel-based quantitative imaging analysis was used to calculate the mean value of PTEN expression level of each voxel,and it displayed on the standard brain atlas through the pseudo-color gradation.Results A total of 75 patients were enrolled in a high PTEN expression group and 68 were enrolled in a low PTEN expression group.There were no significant differences in clinical data,such as age,and sex between the 2 groups (P 〉 0.05).Tumors involving the whole-brain map showed that the glioblastomas in this group of patients often occurred in subependymal region on both sides.The brain atlases of the glioblastoma PTEN expression level showed that the PTEN high expression related brain region mainly located in the right frontal lobe,while the PTEN low expression related brain regions mainly located in the right posterior temporal lobe.Conclusion Quantitative imaging analysis technique can be used to determine the correlation between PTEN expression and anatomic location in glioblastomas,which provides a new evidence for the process of genetic changes in the process of glioblastoma occurrence.
