摘  要:目的 探讨基于二维公式计算和三维重建方法在描述肿瘤大小中的差异.方法 采集2013年1月至2015年3月北京市普仁医院-首都医科大学附属北京天坛医院神经外科合作病房经手术治疗的90例低级别神经胶质瘤患者的磁共振图像.采用二维公式计算方法和三维重建方法测量肿瘤体积.统计检验比较不同方法得出肿瘤体积的差异.结果 在所有病例中,二维公式计算方法和三维重建方法得出的肿瘤体积差异无统计学意义(P =0.158).在前后径与左右径之比>1.2的病例中,二维公式计算方法显著小于三维重建方法得出的肿瘤体积,差异有统计学意义(P=0.002).在三维重建得出的肿瘤体积<60 cm3的病例中,二维公式计算方法显著小于三维重建方法得出的肿瘤体积,差异有统计学意义(P <0.001).结论 二维公式计算方法可以作为肿瘤体积的临床初测方法.但在肿瘤不规则或肿瘤体积较小时,应用公式计算方法估计的肿瘤体积与肿瘤实际体积可能存在偏差,应考虑选择三维重建的方法.Objective To investigate the differences of describing tumor size according to the formula calculation and three-dimensional reconstruction methods.Methods From January 2013 to March 2015,the magnetic resonance images of 90 patients with low-grade glioma treated at the cooperation wards of the Department of Neurosurgeries,Beijing Puren Hospital and Tiantan Hospital,Capital Medical University were collected.The volumes of the tumors were measured by formula calculation and three-dimensional reconstruction methods.Statistical tests were used to compare the differences of tumor volume obtained by differeut methods.Results There was no significant difference in measuring the tumor volume in all cases between the two methods (P =0.158).In 12 patients with a ratio of anteroposterior diameter and transverse diameter 〉 1.2,the tumor volume measured by the formula calculation method was significantly less than that measured by the three-dimensional reconstruction method (P =0.002).In patients whose tumor volume 〈 60 cm3 measured by the three-dimensional reconstruction method,the tumor volume measured by the formula calculation method was significantly less than that of the three-dimensional reconstruction method (P 〈 0.001).Conclusions The formula calculation method can be used as a primary measurement of tumor volume in clinical practice.However,when the tumor is irregular or its volume is smaller,the tumor volume measured by the formula calculation method may have deviation with the tumor actual volume,then,the three-dimensional reconstruction method should be considered.
