CD8^+CD28^-T细胞在胃癌并转移患者的表达及手术切除对其影响探讨

摘　　要：目的:探讨CD8+CD28-T细胞在胃癌并转移患者的表达及手术切除对其影响。方法:纳入58例胃癌患者,采用流式细胞术检测外周血CD8+CD28-T细胞及其细胞因子白介素10(IL-10)及转化生长因子β1(TGF-β1)含量,以及CA72-4含量。术后12周,复诊比较上述指标变化。随访3年,比较转移组(n=30)及未转移组(n=28)的3年生存率,并分析相关性。结果:(1)与健康对照组比较,胃癌组患者CD8+CD28-T细胞、IL-10、TGF-β1及CA72-4均显著升高,差异均有统计学意义(P<0.05);(2)术前,转移组与未转移组的CD8+CD28-T细胞、IL-10、TGF-β1及CA72-4均无统计学差异。术后,未转移组CD8+CD28-T细胞、IL-10、TGF-β1及CA72-4均显著低于术前,且均显著低于转移组(P<0.05)。转移组CD8+CD28-T细胞、IL-10及CA72-4均显著低于术前,而TGF-β1与术前无统计学差异。转移组术后3年内生存率显著低于未转移组;(3)CD8+CD28-T细胞、IL-10、TGF-β1三者当中任意一者,均与CA72-4呈显著正相关性,而与生存率呈显著负相关性。结论:CD8+CD28-T细胞、IL-10及TGF-β1显著升高是胃癌并转移患者的重要免疫表现,三者与肿瘤标记物及生存率密切相关。Objective: To investigate the expression and significance of CD8+CD28-T cells for patients with gastric carcinoma who underwent gastric excision. Methods: Select 58 patients with gastric cancer,using flowcytometry to test peripheral blood CD8+CD28-T cells,cytokine interleukin 10( IL-10),transforming growth factor beta 1( TGF-beta 1) content,and the content of CA72-4. Compare the above indicators 12 weeks after the surgery. Followup for 3 years,compare 3 years survival rate between transfer group( n = 30) and non-transfer group( n = 28),and analyze the correlation. Results:( 1) Compared with healthy control group,CD8+CD28-T cells,IL-10,TGF-β1and CA72-4 in patients with gastric cancer group were significantly elevated,the differences were statistically significant( P < 0. 05);( 2) Before the surgery,CD8+CD28-T cells,IL-10,TGF-β1 and CA72-4 between transfer group and non-transfer group had no statistical difference. After surgery,CD8+CD28-T cells,IL-10,TGF-β1 and CA72-4of non-transfer group were significantly lower than before,and were significantly lower than transfer group( P < 0.05). CD8+CD28-T cells,IL-10 and CA72-4 of transfer group were significantly lower than before,but TGF-β1 had no statistical difference. After surgery,3 year survival rate of transfer group was significantly lower than non-transfer group;( 3) Each one of CD8+CD28-T cells,IL-10,TGF-β1 has significant positive correlation with CA72-4,and has significantly negative correlation with survival rate. Conclusion: Significant rise of CD8+CD28-T cells,IL-10 and TGF-β1 are important immune performance of patients with gastric cancer and are closely related to tumor markers and survival rate.
