CD8^+CD28^-T细胞与胰腺癌患者肿瘤标记物、生存期及治疗的关系

摘　　要：目的:探讨CD8+CD28-T细胞与胰腺癌患者肿瘤标记物、生存期及治疗的关系。方法:以25例胰腺癌患者作为观察对象,以23例慢性胰腺炎及32例健康人作为对照,采集外周血进行CD8+CD28-T细胞及其细胞因子转化生长因子β1(TGF-β1)及白介素10(IL-10)百分含量检测。对胰腺癌患者进行治疗,分别在治疗后第4、8及12周检测外周血CD8+CD28-T细胞、TGF-β1、IL-10及CA199含量,并分析胰腺癌患者CD8+CD28-T细胞及细胞因子与生存期及CA199的关系。结果:胰腺癌组患者CD8+CD28-T细胞、TGF-β1及IL-10三者均显著高于慢性胰腺炎及健康对照组(P<0.05),而后两组之间无统计学差异。治疗后第4周CD8+CD28-T细胞、TGF-β1及IL-10与治疗前无统计学差异,但第8周与第4周有显著性差异,亦即明显降低(P<0.05)。第12周与第8周同样无统计学差异。CD8+CD28-T细胞、TGF-β1及IL-10三者当中任意一者与生存期呈负相关性,但与CA199呈显著正相关性(均为r绝对值大于0.7,P<0.05)。结论:CD8+CD28-T细胞及其细胞因子升高是胰腺癌的特征之一,随着治疗进展其可一度下降,但治疗后期下降不显著;该细胞与生存期及肿瘤标记物密切相关,因而具有重要临床意义。Objective: To investigate the relation of CD8+CD28-T cells with tumor markers,survival time and treatment of patients with pancreatic cancer. Methods: 25 patients with pancreatic cancer were selected as research object,23 cases of chronic pancreatitis patients and 32 cases of healthy people were selected as control objects. Collect peripheral blood to detect the content of CD8+CD28-T cell,TGF-β1 and IL-10. Detect content of CD8+CD28-T cells,TGF-beta 1,IL-10 and CA199 in peripheral blood 4,8 and 12 weeks after treatment. Analyze the relationship of CD8+CD28-T cells and cytokines and survival and CA199. Results: The content of CD8+CD28-T cell,TGF-β1 and IL-10 in pancreatic cancer group were significantly higher than those in chronic pancreatitis group and healthy control group( P < 0. 05),and no statistical difference between chronic pancreatitis group and healthy control group. 4 weeks after treatment,the content of CD8+CD28-T cell,TGF-β1 and IL-10 had no statistical difference,but 8 weeks later the content of CD8+CD28-T cell,TGF-β1 and IL-10 significantly decreased than 4 weeks later( P< 0. 05). 12 weeks later the content of CD8+CD28-T cell,TGF-β1 and IL-10 also had no statistical difference.Each one of CD8+CD28-T cell,TGF-β1 and IL-10 had negative correlation with survival,but significantly positive correlations with CA199( the absolute value of r is greater than 0. 7,P < 0. 05). Conclusion: CD8+CD28-T cells and cytokines rise is one of the characteristics of pancreatic cancer,along with the progress of treatment it may drop,but no significant decline in late treatment. The cells are closely associated with survival and tumor markers,thus has important clinical significance.
