类Y型硅胶管逆行植入术治疗泪道阻塞的疗效观察

摘　　要：目的:观察类Y型硅胶管逆行植入术治疗泪道阻塞的临床效果。方法:泪道阻塞性疾病患者304例分为治疗组(180例,194眼)和对照组(124例,124眼),治疗组行类Y型硅胶管逆行植入术,对照组单纯行机械探通,随访12月,比较其治愈率及复发率。结果:治疗组患者均一次性成功植入Y型硅胶管,术中无并发症发生,术后1周两组患者泪道冲洗均通畅,溢泪症状消失;术后12月治疗组治愈153眼(78.9%),好转29眼(14.9%),复发12眼(6.2%)。对照组治愈37眼(29.8%),好转19眼(15.3%),复发68眼(54.9%)。治疗组治愈率高于对照组而复发率低于对照组,差异有显著意义(P<0.05)。结论:类Y型硅胶管逆行植入术治疗泪道阻塞简单、经济、安全、有效。Objective: To evaluate the effect of retrograde intubations with Y-silicone catheter for treatment of duct obstruction. Methods: 304 patients with duct obstruction were divided into the treatment group( 180 cases,194eyes) and the control group( 124 cases,124 eyes). Retrograde intubation with Y-silicone catheter was performed in the treatment group and probing of lacrimal passage in the control group. Clinical effect was evaluated in 12 months follow-up time. Results: The lacrimal system was able to be irrigated after the operation and epiphora symptoms were eliminated in all patients in the treatment group 1 week after operation. In 12 months follow-up time 153 eyes( 78.9%) were cured,29 eyes( 14. 9%) were improved and no effect was noted in 12 eyes( 6. 2%) in the treatment group. While in the control group 37 eyes( 29. 8%) were cured,19 eyes( 15. 3%) were improved and no effect was noted in 68 eyes( 54. 9%). A significant difference was found in both curative ratio and recurrence rate between 2groups( P < 0. 05). Conclusion: Retrograde intubation with Y-silicone catheter for treatment of duct obstruction is simple,economical,safe and effective.
