二野清扫术和三野清扫术在胸中段食管癌淋巴结中的应用对比

摘　　要：目的：对比二野清扫术和三野清扫术在胸中段食管癌淋巴结中的应用效果.方法：回顾性分析2007年1月-2010年8月某院胸外科手术治疗的189例胸中段食管癌病例,87例行完全二野淋巴结清扫术,102例行完全三野淋巴结清扫术.对比两组手术清扫淋巴结情况、术后并发症发生情况及5年生存率.结果：组间比较三野清扫组的淋巴结清扫总数与平均清扫数均高于二野清扫组,差异具有统计学意义（P 〈0.05）.组间比较三野清扫组术后吻合口瘘与喉返神经损伤发生率均显著高于二野清扫组（P 〈0.05）.组间比较三野清扫组的术后生存率显著高于二野清扫组,差异具有统计学意义（P 〈0.05）.结论：相对于二野清扫术,在胸中段食管癌治疗中应用三野淋巴结清扫术能显著增加淋巴结清扫数,利于减少肿瘤局部复发,提高患者5年生存率,值得在临床推广应用.Objective: To compare the application effect of two-field lymphadenectomy and three-field lymphadenectomy in thoracle esophageal cancer lymph code. Methods: Retrospective analyze 189 cases of patients with thoracle esophageal cancer in a hospital from January 2007 to August 2010,among which,87 cases received complete twofield lymphadenectomy and 102 cases received complete three-field lymphadenectomy. The lymph node lymphadenectomy condition,postoperative complications,and 5-year survival rate of patients in both groups were compared. Results: The total and average lymphadenectomy numbers of three-field lymphadenectomy group were higher than those of the two-field lymphadenectomy. The differences were statistically significant( P < 0. 05). After lymphadenectomy,the anastomotic fistula and laryngeal recurrent nerve injury rate of the three-field lymphadenectomy were significantly higher than those of the two-field lymphadenectomy group( P < 0. 05). The postoperative survival rate of the threefield lymphadenectomy group was significantly higher than that of the two-field lymphadenectomy. The difference was statistically significant( P < 0. 05). Conclusion: Compared to two-field lymphadenectomy,the application of threefield lymphadenectomy on thoracle esophageal cancer lymph node can significantly increase the lymphadenectomy number,facilitate the recurrence of local tumor,and improve the 5-year survival rate,which is worth clinical promotion.
