氟达拉滨联合中剂量阿糖胞苷治疗复发难治性急性髓系白血病41例临床疗效分析

摘　　要：目的探讨氟达拉滨联合中剂量阿糖胞苷治疗复发难治性急性髓系白血病效果。方法选择2010年5月～2015年5月期间我院收治的复发难治性急性髓系白血病（rrAML）患者81例作为研究对象，按照治疗方法分为两组，观察组41例，对照组40例；对照组用氟拉达滨联合大剂量阿糖胞苷方案（FLAG）治疗，观察组患者用CAG预激方案治疗，比较两组患者临床疗效及不良反应发生情况。结果观察组有效率高于对照组（68．29％vs45．00％），两组差异有统计学意义（P〈0．05）。两组感染率比较差异有统计学意义（78．05％vs52．50％，P〈0．05）；观察组口腔毒性发生率为14．63％，显著低于对照组的55．00％，差异有统计学意义（P〈0．05）。两组TRM比较差异无统计学意义（5．00％vs7．32％，P〉0．05）。结论复发难治性急性髓系白血病患者用氟达拉滨联合中剂量阿糖胞苷治疗效果较好，降低口腔毒性反应发生的风险，有较高的安全性。Objective To investigate the effect of fludarabine and moderate dose cytosine arabinoside in treatment of patients with refractory and relapse acute myeloid leukemia. Methods A total of 81 patients with relapsed and refractory acute myeloid leukemia （rrAML） were selected from May 2010 to May 2015 period in our hospital, all the patients were divided into two groups according to the treatment, there were 41 cases in the observation group and 40 cases in the control group, the control group was treated with a fluorine rada bin joint high-dose cytosine arabinoside （FLAG）, the observation group was treated with CAG WPW regimen, the clinical efficacy and adverse events of two groups were comapred. Results The effect rate of the observation group （68.29%） was higher than the control group （45.00%）, there was significant difference between two groups（P〈0.05）. There was significant differences in the infections rate between two granps（78.05% vs 52.50%, P〈0.05）; The oral toxicity in the observation group （14.63%） was lower than the control group（55.00%）, there was significant difference between two groups （P〈0.05）. There was no significant difference in TRM between two groups（5.00% vs 7.32%, P〉0.05）. Conclusion The effect of relapsed refractory acute myeloid leukemia patients treated with fludarabine-based moderate dose cytosine arabinoside therapy is better, and it can reduce the risk of oral toxicities, the security is higher.
