眼眶爆裂性骨折Medpor修复术效果欠佳原因分析

摘　　要：目的分析眼眶爆裂性骨折高密度多孔聚乙烯（Medpor）修复术后效果欠佳情况及其原因。方法对我科收治的眼眶爆裂性骨折行Medpor修复术后出现效果欠佳的患者进行了回顾性分析。结果在232例术后随访3个月中：2例视功能明显下降，7例眼球内陷欠矫，2例残留复视，植入物移位1例。结论眼眶爆裂性骨折行Medpor修复术后不良效果较多，可以出现视力下降、眼球内陷欠矫、复视加重、感染等。其原因多与受伤程度、部位、术者技术手法及术后功能训炼有关，预防并及时处理术后出现的不良情况，查找原因给予及时治疗。是眼眶骨折修复手术成功的关键。Objective To analyze the causes of complications after orbital reconstruction with high density porous polyethylene （Medpor） for the repair of orbital blowout fracture. Methods A retrospective analysis of patients with orbital blowout fracture treated with Medpor in our department was carried out. Results In 232 cases after postoperative follow up of 3 months, the visual function decreased significantly in 2 cases, endophthalmos was undercorrection in 7 cases, residual diplopia was observed in 2 cases and implant displacement was detected in 1 case. Conclusion There are many kinds of complications after orbital reconstruction with Medpor implantation for blow-out fracture of orbital floor wall, such as vision loss, enophthalmos, diplopia exacerbation, infection, and so on. The causes are related to the degrees of fracture, location, doctor＇ s skill and experience, and postoperative functional training. The key points for successful repair of orbital fracture include effective prevention, correct analysis of causes and timely treatment.
