角膜穿孔伤伴组织缺损的修复手术方法

摘　　要：目的评价巩膜片移植或联合自体角膜缘移植修复周边角膜穿孔并角膜组织缺损的临床疗效。方法采用回顾性病例研究，观察我院1992年8月至2013年2月收治的角膜周边穿孔伤伴有组织缺损12例（12眼）。其中7只眼进行自体伤眼游离板层巩膜移植修补术，另5眼进行保存异体巩膜移植联合自体角膜缘移植术。术后随访至少12个月。观察愈合情况、术后视力及并发症。结果两种手术方法12眼术后穿孔均封闭，随访期内未见明显并发症，且视力均得到了一定程度的恢复。结论巩膜移植或联合自体角膜缘移植修复周边角膜穿孔伤及组织缺损，可作为应急处理的方法。Objective To evaluate the feasibility and clinical efficacy of scleral graft or combined with autologous limbal transplantation for the treatment of peripheral corneal perforation with defect of tissue. Methods Twelve eyes of 12 inpatients with peripheral corneal perforating wound in our hospital from August 1992 to February 2013 were retrospectively analyzed. Seven patients received scleral graft transplantation only, and 5 patients received preserved scleral graft combined with autologous limbal transplantation. The follow-up period was at least 12 months after the transplantation. Postoperative visual acuity and complications were studied in the follow-up period. Results All perforations （ 12 eyes） were closed, no significant complication occurred during follow-up period, and visual functions were improved. Conclusion Either scleral graft or combined with autologous limbal transplantation for the treatment of peripheral corneal perforation may be a new effective method for the treatment of emergent peripheral corneal perforation and defect.
