乳腺癌术后辅助化疗对外周血Caspase-3、Survivin表达的影响

摘　　要：目的探讨乳腺癌术后辅助化疗对Caspase-3、存活素(Survivin)mRNA表达的影响及辅助化疗的意义。方法接受乳腺癌切除术的患者52例,随机数字表法分为化疗组(30例)和未化疗组(22例)。其中化疗组化疗方案为4EC-4T(环磷酰胺+表柔比星;序贯多西紫杉醇,各4个周期)。比较两组患者外周血中Caspase-3、Survivin mRNA在术前1 d和术后190 d的表达水平。结果术前外周血中Caspase-3、Survivin表达水平差异无统计学意义(P>0.05)。术后化疗结束后,Survivin表达水平较术前明显下降(P<0.05),且化疗组下降更显著(P<0.05);Caspase-3表达水平较术前明显升高(P<0.05),且化疗组升高更显著(P<0.05)。结论乳腺癌术后辅助化疗可增加Caspase-3和降低Survivin表达水平,提示术后化疗抗肿瘤机制可能与特异性促进癌细胞凋亡,抑制肿瘤细胞微转移有关,外周血Caspase-3、Survivin表达水平可能在一定程度上反映乳腺癌细胞活动状态及患者预后情况。Objective To explore the effect and significance of Caspase-3 and Survivin in peripheral blood in breast cancer patients treated with adjuvant chemotherapy after mammectomy. Methods 52 patients with breast cancer resection were divided into chemotherapy group（ 30 cases） and non chemotherapy group（ 22 cases）. The chemotherapy regimen was 4EC-4T（ cyclophosphamide ＋ epirubicin; sequential docetaxel,4 cycles）. The expression level of Survivin and Caspase-3 mRNA in peripheral blood one day before operation and 190 th day after operation were detected and compared between two groups. Results There was no significant difference in the expression level of Caspase-3 and Survivin between two groups before operation（ P 〉 0. 05）. At 190 days after operation,the expression level of survivin decreased significantly compared with the preoperative（ P 〈 0. 05） and the decrease in the chemotherapy group was more significant（ P 〈 0. 05）. The expression level of Caspase-3 increased significantly compared with the preoperative（ P 〈 0. 05） and the increase in the chemotherapy group was more significant（ P 〈 0. 05）. Conclusion Adjuvant chemotherapy can increase Caspase-3 and decrease Survivin expression level in breast cancer patients,which may be related to tumor cell apoptosis and inhibit tumor cell apoptosis. The expression of Survivin and Caspase-3 may reflect the activity state and prognosis of breast cancer.
