中心性浆液性脉络膜视网膜病变光动力疗法与激光治疗的疗效观察

摘　　要：目的观察激光与光动力疗法(photdynamic therapy,PDT)治疗中心性浆膜性脉络膜视网膜病变的临床疗效。方法将44眼经眼底荧光血管造影确诊为中心性浆液性脉络膜视网膜病并且渗漏点与黄斑中心凹无血管区之间距离>250μm的患者,根据病人意愿分为激光治疗组(A组)和PDT治疗组(B组)各22眼,A组采用黄激光(576nm),B租采用1/2常规剂量PDT治疗。定期观察治疗后最佳矫正视力,Amsler方格表、黄斑光学相干断层扫描(OCT)等。结果 PDT治疗组在术后各个随访时间达到治愈的病例数均较激光治疗多,差异有统计学意义。结论光动力疗法治疗能明显缩短病程,是一种治疗中心性浆膜性脉络膜视网膜病的有效方法。Objective To observe the effect of photodynamic therapy（PDT） with 1/2-doses verteporfin and laser for central serous retinopathy. Methods To chose CSC patients with lesions located 250μm out macular foveal diagnosed by OCT and FFA, and divide to into two groups, A and B. A with laser therapy,22 eyes were enrolled. B treated using PDT with 1/2-dose verteporf in,22 eyes were enrolled. BCVA and OCT and Amsler Grid were performed before and 2week,1 month,3 months and 6 month after treatment.Results The total effectiveness in A group was lower than in B group in every time point. The average best corrected visual acuity（BCVA） was improved from 0.52 to 0.88 in B group, compared with the A group was from 0.54 to 0.70, the difference was statistically significant（P〈0.05）.Conclusion Photodynamic therapy with 1/2-dose verteporfin appears to be an effective and safe treatment for patients with chorioretinopathy.
