摘  要:目的探讨影响放射治疗胶质瘤患者预后的因素。方法胶质瘤患者105例术后接受60℃射线放射治疗,生存率分析采用KaplanMeier法,生存曲线比较采用Log-rank法,采用COX比例风险回归方程分析患者预后的影响因素。结果随访4.31~94.80个月,中位随访期52.79个月。共有37例存活,死亡68例,死亡率为66.67%;单因素分析结果显示:年龄、手术切除程度、病理类型、手术放疗时间和术前Karnofsky功能状态评分(KPS)是影响胶质瘤术后放疗疗效的相关因素;Cox比例风险回归模型分析结果显示:影响胶质瘤患者预后的影响因素包括年龄(RR=3.021,P<0.05)、病理分级(RR=3.417,P<0.05)、KPS(RR=0.542,P<0.05)和手术切除程度(RR=1.685,P<0.05)。结论年龄、病理分级、手术切除程度和KPS是影响放射治疗胶质瘤预后的影响因素,胶质瘤仍以手术治疗为主,术中应尽量完整切除肿瘤,术后尽早辅以放化疗。Objective To investigate the prognostic factors of radiotherapy in patients with glioma. Methods 105 cases of glioma patients received postoperative^(60)Co beam radiation therapy,survival analysis used Kaplan-Meier method,survival curves were compared using Log-rank method,prognostic factors were analyzed using COX proportional hazards regression analysis. Results All patients were followed up for 4.31 to 94.80 months,with a median follow-up of 52.79 months. 37 patients survived and 68 died,the mortality rate was 66.67%; univariate analysis showed: age,extent of surgery,pathology,surgery time and preoperative radiotherapy KPS scores were influential factors of glioma after radiotherapy related factors; Cox proportional hazards regression analysis showed that: factors influencing the prognosis of glioma patients included age( RR = 3.021,P〈0.05),pathological grade( RR = 3.417,P〈0.05),KPS score( RR = 0.542,P〈0.05),and extent of surgical resection( RR = 1.685,P〈0.05).Conclusions Age,tumor grade,extent of surgical resection and KPS scores are factors influencing the prognosis of glioma radiation therapy,the treatment of glioma is still surgery,surgery should be complete resection of the tumor,post-operation combining with radiotherapy and chemotherapy as soon as possible.
