摘 要:目的 分析甲状腺乳头状癌(papillary thyroid carcinoma,PTC)肿瘤组织与癌旁正常组织的代谢组学特征,以及代谢产物与PTC原发灶大小、淋巴结转移的关系.方法 采集2014年3月至6月在上海交通大学医学院附属仁济医院头颈外科接受手术治疗的43例女性PTC患者肿瘤组织及其癌旁正常组织,联合应用气相色谱-飞行时间质谱和超高效液相色谱-飞行时间质谱技术检测癌组织和癌旁正常组织内的代谢产物,分析PTC的代谢组学特征,根据原发灶大小、有无颈部淋巴结转移,将病例分为T1组(原发灶最大径≤2.0 cm)和T2组(原发灶最大径>2.0 cm)、N-(无淋巴结转移)组和N+组(有淋巴结转移).比较各组间肿瘤组织内代谢产物的差异.结果 女性PTC组织与癌旁正常组织间共存在46种差异代谢产物,其特征为癌组织内氨基酸代谢、嘌呤与嘧啶代谢、色氨酸代谢、一碳单位代谢、糖酵解代谢、牛磺酸与亚牛磺酸代谢途径增强.肿瘤组织内代谢产物在T1组和T2组间差异表现为T2组内氨基酸、嘌呤与嘧啶、色氨酸以及一碳单位代谢较T1组增强;肿瘤组织内代谢产物在N-组与N+组间差异表现为N+组内糖酵解、氨基酸代谢、嘌呤与嘧啶、色氨酸、一碳单位代谢较N-组增强.结论 女性PTC组织存在特异性的代谢产物,并且这些代谢产物与PTC肿瘤直径、有无颈部淋巴结转移相关.Objective To analyze the metabolic profiles of the female papillary thyroid carcinoma (PTC) and the relationship between the metabolic profiles and primary tumor size and cervical lymph node metastasis using a metabolomics approach.Methods Forty-three cases of female PTC were enrolled in this study.Gas chromatography-time-of-flight mass spectrometry and ultra performance liquid chromatography-time of flight-mass spectrometry were employed for analyzing metabolic profiles of tumor tissues and adjacent normal tissues in the female PTC.Cases were divided into Group T1 (tumor size ≤2.0 cm) and Group T2 (tumor size 〉 2.0 cm) according to the tumor size,and divided into Group N-(with negative cervical lymph node) and Group N + (with positive cervical lymph node) according to the cervical lymph node conditions.We compared the metabolomic profiles between these groups.Results A panel of 46 differentially expressed metabolites was identified in the PTC specimens,compared with normal tissues.Increased metabolism of amino acid,purine and pyrimidine,tryptophan acid,one carbon,glycolysis,taurine and hypotaurine,and fatty acid were found in PTC tumors tissues.Amino acids,purine and pyridine,tryptophan,and carbon metabolism increased significantly in the tumor tissues of Group T2 compared with Group T1,while glycolysis,amino acid,purine and pyridine,tryptophan,and carbon metabolism increased significantly in the specimens of Group N +.Conclusion Distinct metabolic profiles were identified in the female PTC tissues,which were related to the primary tumor size and cervical lymph node metastases.
