腹腔镜下根治性远端胃癌切除术+Roux-en-Y胃转流术治疗早期胃癌合并肥胖型2型糖尿病的治疗体会

摘　　要：目的:探讨早期胃癌合并肥胖型2型糖尿病的治疗方式。方法:回顾收治的1例典型病例的临床资料,并查阅相关文献进行分析。结果:患者术后第11天出院,出院后30 d门诊随访,一般情况尚可,停用胰岛素及降糖药物,空腹血糖6.0 mmol/L,糖化血红蛋白5.9%。结论:早期胃癌合并肥胖型2型糖尿病患者行腹腔镜下根治性远端胃癌切除术+Roux-enY胃转流术可实现肿瘤与代谢疾病的同时治疗。Objective： To explore the treatment of early gastric cancer with obesity and type 2 diabetes. Methods： The clinical data of 1 classic patient of early gastric cancer with obesity and type 2 diabetes was reviewed and relevant literatures were analyzed. Results： The patient was discharged from hospital on the 11 th day after operation,and followed up on the 30 th day after discharge in outpatient clinic. Insulin and hypoglycemic agents were stopped,fasting blood glucose was 6. 0 mmol / L,glycosylated hemoglobin was 5. 9%.Conclusions： Early gastric cancer with obesity and type 2 diabetes could be treated together by laparoscopic radical gastrectomy with Roux-en-Y gastric bypass. For these patients,laparoscopic radical gastrectomy with Roux-en-Y gastric bypass could be a choice.
