上消化道出血合并高原红细胞增多症临床治疗分析

摘　　要：目的:分析上消化道出血合并高原红细胞增多症患者胃镜检查结果、幽门螺杆菌感染等状况。方法:收集符合纳入排除标准的上消化道出血合并高原红细胞增多症患者28例,取同期入院治疗的高原地区非高原红细胞增多症并伴有上消化道系统症状的患者28例作为对照组,对两组胃镜检查结果、幽门螺杆菌感染状况进行对比分析。结果:高原红细胞增多症合并上消化道出血患者胃溃疡、十二指肠溃疡、急性胃粘膜病变发生率分别为35.7%、32.1%、35.7%,对照组分别为10.7%、7.1%、3.6%,两组分别比较,合并组高于对照组,差异有统计学意义。其中高原红细胞增多症合并上消化道出血组患者中世居高原患者急性胃粘膜病变高于移居患者,对照组中胃溃疡、十二指肠溃疡、急性胃粘膜病变发生率移居和世居患者差异不具有统计学意义。上消化道出血合并高原红细胞增多症患者幽门螺杆菌检测阳性率高于对照组。结论:上消化道出血合并高原红细胞增多症患者胃溃疡、十二指肠溃疡、急性胃粘膜病变、幽门螺杆菌检测阳性率高。Objective To analysis the high plateau polycythemia combined with upper gastrointinal bleeding.Methods：Sellected 28 cases suffer from high plateau polycythemia combined with upper gastrointinal bleeding and 28 patients suffer from the UGI disease who live in same place as the control group.Made comparative analysis of the conclusion of gastroscopy and HP.Results The incidence of gastric ulcer,duodenal ulcer,acute gastric mucosal lesions was 35.7%,32.1%,35.7%in patients of the high plateau polycythemia combined with upper gastrointinal bleeding,which is higher than the control group＇s incidence（10.7%、7.1%、3.6%）the differences were statisticaly significant.In the group of the high plateau polycythemia combined with upper gastrointinal bleeding,the patients living in plateau for a long time was higher than migration in acute gastric mucosal lesions,In the group of the control,the difference between patients living in plateau for a long time and migration patients was not Significant in gastric ulcer,duodenal ulcer and acute gastric mucosal lesion.The patient of high plateau polycythemia combined with upper gastrointinal bleeding is higher than control group in positive incidence of Helicobacter pylori.Conclusion the group of high plateau polycythemia combined with upper gastrointinal bleeding is higher than that of the control group in incidence of gastric ulcer,duodenal ulcer,acute gastric mucosal lesion,Helicobacter pylori.
