N—Myc下游调节基因3在胃癌组织的表达及临床意义

摘　　要：目的观察N—Mye下游调节基因3（NDRG3）在胃癌组织中的表达并探讨其临床意义。方法采用免疫组织化学法检测120例胃癌患者癌组织及其相应的癌旁组织中NDRG3的表达，分析其与患者临床病理特征和预后的关系。结果癌组织中NDRG3的高表达率为43．3％（52／120），显著高于正常胃组织。癌组织中NDRG3高表达与患者幽门螺杆菌（Hp）感染状态（x^2=21．08，P〈0．01）、肿瘤分化程度（x^2=6．96，P〈0．05）、浸润深度（x^2=4．81，P〈0．05）、淋巴结转移状态（x^2=13．64，P〈0．01）、TNM分期（x^2=22．51，P〈0．01）具有显著相关性，而与患者性别、年龄、肿瘤部位和大小无明显相关（P〉0．05）。此外，Kaplan—Meier生存分析显示，NDRG3高表达的胃癌患者与正常／低表达患者中位生存时间分别为28个月和68个月，两者差异有统计学意义（P〈0．05）。Cox多因素回归分析表明NDRG3高表达是胃癌患者独立预后因素[风险比（HR）=2．49，P〈0．05]。结论NDRG3在胃癌组织中表达上调，与胃癌发生进展及预后明显相关。Objective To investigate the expression level of N - myc downstream regulated gene （NDRG3） in gastric cancer tissues and the clinicopathologic significance. Methods The expression level of NDRG3 in 120 pairs of gastric cancer tissues and corresponding normal tissues was detected by immunohistochemistry. The association of NDRG3 expression level with clinicopathologic characteristics and prognosis of grastric cancer patients was analyzed with SPSS 21 software. Results The NDRG3 in gastric cancer tissues was highly expressed in 52 of 120 （43.3%） gastric cancer tissues, and significantly upregulated as compared with their paired normal tissues （0% ,x^2 = 66. 38, P 〈 0. 05 ）. In addition, the high expression of NDRG3 in gastric cancer tissues was significantly correlated with H. pylori infection status （x^2= 21.08, P 〈 0. 01 ）, tumor differentiation （x^2= 6. 96, P 〈 0. 05 ）, invasive depth （x^2 = 4. 81,P 〈 0. 05 ）, nodal metastasis status （x^2= 13.64,P 〈0. 01 ） and TNM stage （x^2 =22. 51 ,P 〈0.01 ）, but not with gender, age, distant metastasis, tumor location and size （ P 〉 0. 05）. Furthermore, Kaplan - Meier analysis showed that there was significant difference in overall survival （OS） between the patients with high or normal/low expression of NDRG3 in gastric cancer tissues （ median survival time was 28 months and 68 months respectively, P 〈 0. 05）. The result of multivariate Cox analysis revealed the high expression of NDRG3 was an independent factors for gastric cancer patients [ hazard ratio （HR） - 2. 49, P 〈 0. 01 ]. Conclusion The expression of NDRG3 in gastric cancer was upregulated and correlated with tumorigenesis, progress and prognosis for grastric cancer.
