摘  要:目的分析新疆哈萨克族(哈族)及汉族食管鳞状细胞癌(鳞癌)中白细胞分化抗原44变异体5(CIM4v5)表达与临床病理特征之间的关系,探讨血清CD44v5含量检测用于食管鳞癌筛检的意义。方法收集我院胸外科手术切除食管鳞癌标本哈族、汉族各50例,采用反转录一聚合酶链反应(RT—PCR)法检测CD44v5mRNA在癌组织及癌旁组织中的表达;并用Western blot法对CD44v5蛋白表达进行半定量测定;采用酶联免疫吸附法(ELISA)测定该蛋白在上述食管鳞癌息者及60例健康体检者血清的含量。结果(1)CD44v5mRNA及其编码的蛋白在癌组织中表达(0.737±0.116、1.099±0.230)均高于癌旁组织(0.140±0.065、0.265±0.105),CD44v5蛋白在食管鳞癌患者血清中的表达水平[哈族:(30.575±10.379)μg/L,汉族:(32.041±1.680)μg/L]高于健康体检者水平(19.364±1.680)μg/L,差异均有统计学意义(P〈0.01)。(2)CDd4v5mRNA在ⅡB+Ⅲ期组癌组织中较Ⅰ+ⅡA期组中的相对表达量上调(P〈0.01),有淋巴结转移组癌组织中的相对表达量高于无淋巴结转移组(P〈0.01),CD44v5蛋白在ⅡB+Ⅲ期患者表达量[组织:1.222±0.176;血清:(37.137±7.316)μg/L]中高于Ⅰ+ⅡA期患者[组织:0.862±0.097;血清:(19.994±1.460)μg/L];有淋巴结转移组[组织:1.309±0.072;血清:(40.762±3.161)μg/L]也高于无淋巴结组[组织:0.882±0.093;血清:(21.468±2.668)μg/L,P〈0.01]。(3)CD44v5基因表达在不同民族、性别、年龄分组中差异无统计学意义(P〉0.05)。结论CD44v5表达与哈族和汉族食管鳞癌的发生、浸润转移及临床分期(TNM分期)有关。Objective To analyze the expression of CD44v5 and relationship with clinic pathological significance in Kazakh and HaM esophageal squamous cell carcinoma (ESCC) ; Then explore the assessment and screening application value of CD44v5 content in serum detection for esophageal squamous carcinoma. Methods Collect the specimen ( Kazakh the haM 50 cases each) surgical removal of our hospital thoracic: between from January 2010 to December 2012. Postoperative pathology confirmed for squamous cell carcinomas. To detect the CIM4v5 mRNA and protein in Kazakh and HaM ESCC tissues and its corresponding adjacent non - carcinomatous tissues by reverse transcriptase - polymerase chain reaction (RT- PCR) and Western blotting; and use enzyme -linked immunosorbent assay (ELISA) to detect the protein in serum of ESCC patients (case group) and 60 healthy cases (control group). Results (1)The expression of CD44v5 mRNA and CD44v5 protein in carcinoma tissue (0. 737 ±0. 116, 1. 099 ±0. 230) is higher than corresponding adjacent tissues (0. 140± 0. 065, 0. 265 ±0. 105 ) with statistically significant, and the protein in the serum of patients [ Kazakh: (30.575± 10. 379)μg/L, HaM: (32.041±1. 680) btg/L] is higher than heahhy controls too ( 19. 364± 1. 680)μg/L (P 〈0.01 ). (2)The expression of CD44v5 mRNA in Ⅱ B +Ⅲ stage is higher than this of Ⅰ + Ⅱ A (P 〈 0. 01 ). The level with lymph node metastasis is higher than those without. The expression of CD44v5 protein in ⅡB + Ⅲ stage [ issues : 1. 222 ±0. 176; serum: (37. 137 ±7. 316) μg/L] is higher than this of Ⅰ + Ⅱ A [issues: 0. 862 ± 0. 097 ; serum: ( 19. 994± 1. 460) μg/L, P 〈0. 01 ]. The level of CD44v5 protein in patients with lymph node metastasis [ issues: 1. 309± 0. 072; serun: (40. 762 ± 3. 161 ) μg/L] is higher than that without lymph node metastasis [ issues: 0. 882 ±0. 093 ; serum: (21. 468 ±2. 668 ) μg/L, P 〈 0. 01 ].
