乳腺黏液癌超声造影的定性分析

摘　　要：目的探讨乳腺黏液癌超声造影的定性特征。方法分析经病理确诊的乳腺黏液癌病灶的超声造影的定性特征。结果入组病例13例,其中单纯型11例、混合型2例,12例(92.31%)增强水平为高增强;10例(76.92%)增强方向为向心性增强;10例(76.92%)增强形式为不均匀增强;9例(69.23%)增强后病灶边界清晰;10例(76.92%)病灶的增强后范围增大,该10例病灶的左右径均不同程度增大,其中7例(53.85%)同时伴有前后径增大;8例(61.54%)病灶内可见灌注缺损,仅1例(7.69%)病灶周边见蟹足样增强。结论乳腺黏液癌的超声造影表现与其他乳腺良、恶性病灶间均存在一定重叠,对造影后边界清晰、范围向两侧及后方增大并伴有灌注缺损的实性或混合性病灶应警惕黏液癌的可能。Objective To investigate the qualitative characteristics of contrast enhanced ultrasound in breast mucinous carcinoma.Methods Those patients with breast mucinous carcinoma confirmed by pathology were analyzed retrospectively and qualitatively,which had contrast enhanced ultrasound examination before operation.Results In total,two mixed type and eleven pure type breast mucinous carcinoma were enrolled.Hyper-enhancement was observed in 12cases（92.31%）,centripetal and heterogeneous enhancement were both observed in 10cases（76.92%）,well-defined margins were identified in 9cases（69.23%）,transverse diameter expansion was identified in 10cases（76.92%）and anteroposterior diameter expansion in 7cases（53.85%）,focal perfusion defects were present in 8cases（61.54%）,claw-shaped enhancement was detected in only one case（7.69%）.Conclusions Breast mucinous carcinoma has its own characteristics of contrast enhanced ultrasound,and there is a certain overlap between mucinous carcinoma and benign lesions.Those solid or mixed lesions with lager scope,well-defined margins and focal perfusion defects could be breast mucinous carcinoma which should be vigilant.
