胸腔镜辅助非小细胞肺癌手术的近期疗效

摘　　要：目的总结胸腔镜辅助非小细胞肺癌(NSCLC)手术的近期疗效。方法Ⅰa~Ⅱa期NSCLC患者75例行肺癌根治术。其中,胸腔镜辅助手术(VATS)组36例,开胸手术(OS)组39例。比较两组手术相关指标,评估术后6个月内肺功能和肺癌患者生存质量测定量表(FACT-L)评分。结果与OS组相比,VATS组术中出血量和术后第1天胸腔引流量少,术后首次下床时间早,住院时间短,术后3d内血浆超敏C反应蛋白水平低,术后6个月内肺功能较好,FACT-L评分高(P<0.05)。结论与开胸手术相比,胸腔镜辅助行Ⅰa~Ⅱa期NSCLC手术治疗创伤小,对肺功能损害小,生活质量较高。Objective To summarize the short-term efficacy of video-assisted thoracic surgery for patients with stage Ⅰa-Ⅱa non-small cell lung cancer（NSCLC）.Methods Seventy-five patients with stageⅠa-Ⅱa NSCLC were divided into group A（underwent video-assisted thoracic surgery,36cases）and B（underwent open surgery,39cases）.The surgery-related indicators of two groups were compared.The pulmonary function and quality of life were evaluated by functional assessment of cancer therapy-lung（FACT-L）scale in 6months after operation.Results Compared with group B,the patients in group A had less blood loss in surgery and drainage on the 1^（st） postoperative day,earlier ambulation time,shorter hospital stay,and lower plasma level of high-sensitivity C-reactive protein（P〈0.05）.The pulmonary function and quality of life were better in group A than those in group B in6 months after operation（P〈0.05）.Conclusion Compared with open surgery,video-assisted thoracic surgery has the advantages of less surgical trauma,less damage of lung function and better quality of life in the treatment of patients with stageⅠa-Ⅱa NSCLC.
