3D-DSA评估颈内动脉-后交通动脉动脉瘤形成的形态学特征

摘　　要：目的分析颈内动脉眼动脉段与后交通动脉长轴方向的夹角（Apcom）、颈内动脉眼动脉段和后交通段的夹角（Aica）、颈内动脉后交通段与后交通动脉长轴的夹角（Apcom’）大小对颈内动脉-后交通动脉动脉瘤形成的影响。方法回顾性分析24例颈内动脉-后交通动脉动脉瘤（动脉瘤组）与22例非脑动脉瘤（对照组）的临床资料。通过三维血管重建技术获得满意的三维血管立体图像,从而测量其血管夹角参数。结果两组间Apcom、Aica和Apcom’、性别和年龄差异具有统计学意义（P〈0.05）。经Logistic回归分析显示仅Apcom（P=0.008）和Aica（P=0.007）与动脉瘤的发生具有独立相关性。Apcom曲线下面积（area under ROC curve,AUC）为0.799（95%CI：0.661～0.938）,最佳诊断界值为100.2°。Aica的AUC为0.764（95%CI：0.622～0.906）,最佳诊断界值为113.3°。结论 Apcom和Aica的大小与颈内动脉-后交通动脉动脉瘤形成具有相关性。Objective To analyze the formation of internal carotid-posterior communicating artery aneurysm by detecting the size of the angle between the ophthalmic segment and origin of the posterior communicating artery（Apcom）, the angle between ophthalmic and communicating segment of the internal carotid artery（Aica） and the angle between the communicating segment of the internal carotid artery and origin of the posterior communicating artery（Apcom＇） with 3D-DSA. Methods The clinical data of 24 patients with internal carotid-posterior communicating artery aneurysm（aneurysm group） and 22 patients without any aneurysm（control group） were analyzed retrospectively. The 3-dimensional models of involved arteries were constructed for analyzing the morphological features. Results Statistically significant differences were found in the value of Apcom, Aica, Apcom＇, sex and age between the two groups（P〈0.05）.Only Apcom（P = 0.008） and Aica（P = 0.007） were independent associations with the formation of internal carotid-posterior communicating artery from the logistic regression. The area under ROC curve（AUC） of Apcom was 0.799（95% CI： 0.661～0.938）, and the threshold for optimal diagnosis was 100.2°. The AUC of Aica was 0.764（95%CI： 0.622～0.906）, and the threshold for optimal diagnosis was 113.3°.Conclusion The formation of internal carotid-posterior communicating artery aneurysm may be associated with the sizes of Apcom and Aica.
