ICU内脑出血患者肺部物理治疗疗效评估

摘　　要：目的探讨ICU内脑出血患者肺部感染物理治疗的疗效。方法选择2012~2014年我科室收治的确诊为脑出血的患者作为研究对象,随机分为实验组和对照组,其中实验组52例,对照组52例,对照组行常规手术治疗,实验组在对照组的基础上行肺部物理治疗,对比两组患者肺部感染率、住院时间、呼吸机使用时间及指脉血氧饱和度的变化。结果经治疗后,实验组肺部感染率及住院时间为3.86%和12.5±2.0,显著低于对照组的17.31%和21.5±2.1,差异均有显著意义（P〈0.05）;实验组呼吸机使用天数4.50±1.21,较对照组6.30±1.73缩短,实验组指脉血氧饱和度95.10%±1.10%,较对照组92.20%±1.09%升高,差异均有统计学意义（P〈0.05）;实验组治疗的优良率为92.31%,显著高于对照组67.31%,差异有统计学意义（P〈0.05）。结论对ICU内患者进行物理治疗,疗效显著,可减低患者肺部感染率、住院时间及呼吸机使用时间,值得临床推广。Objective To explore the influence and relative safety of physiotherapy on the lung infections of patients with cerebral hemorrhage in ICU. Methods Totally 104 cases of senile cerebral hemorrhage patients treated in our hospital from 2012 to 2014 were selected. They were randomly divided into control group（ n = 52） and observation group（ n = 52）. General treatment was used in the control group,and on the basis of that,pulmonary physiotherapy treatment was adopted in the observation group. The life time of the breathing machine,blood oxygen saturation（ Sa O2）,length of hospital stay and lung infection rates were compared between the two groups. Results After the treatment,the pulmonary infection rate and hospital stay of observation group were 3. 86% and（ 12. 5 ± 2. 0） d respectively,which were significantly lower than those of control group of 17. 31% and（ 21. 5 ± 2. 1） d（ P〈0. 05）. The life time of the breathing machine was（ 4. 5 ± 1. 21） d in the observation group,which was lower than those of control group of（ 6. 3 ± 1. 73） d,and the changes of Sa O2was（ 95. 1 ± 1. 10） % of observation group,which was higher than the control group of（ 92. 2 ± 1. 09） %（ P〈0. 05）.The excellent and good rate of observation group was 92. 31%,which was significantly higher than the control group of 67. 31%（ P〈0. 05）. Conclusion Lung physical therapy in patientsin ICU with cerebral hemorrhage can significantly reduce pulmonary infection rate,length of time,and the life time of breathing machine,the curative effect is remarkable. It is worth to be widely used in clinical practice.
