手术切除与射频消融治疗符合米兰标准的原发性肝癌的疗效对比分析

摘　　要：目的分析手术切除与射频消融治疗符合米兰标准的原发性肝癌的有效性和安全性。方法回顾性分析2010年12月-2012年7月在解放军302医院行手术切除或射频消融治疗的105例符合米兰标准的原发性肝癌患者临床资料,比较两组术后生存率、无瘤生存率、并发症及住院天数。结果 105例患者分为单发单纯组73例,多发联合组32例。分析结果表明,单发单纯组与多发联合组手术切除术后3年总生存率及总无瘤生存率均优于射频消融治疗(P<0.05),射频消融治疗后主要并发症发生率明显低于手术治疗(P<0.05),且射频消融术后具有住院时间短、恢复快等优势。结论符合米兰标准的原发性肝癌可从外科手术中获益,且远期疗效优于射频消融治疗。Objective To analyze the effectiveness and safety of surgical resection and radiofrequency ablation therapy in treatment of primary liver cancer meeting the Milan criteria. Methods The clinical data of 105 cases of primary liver cancer meeting the Milan criteria in 302 Hospital from Dec. 2010 to Jul. 2012 who were treated respectively by surgical resection or radiofrequency ablation therapy were retrospectively rev iewed. The postoperative sur v ival rate, tumor-free sur v ival rate, complications, and hospital stay days were compared between two groups. Results Patients were divided into single pure group（n=73） and multiple combined group（n=32）. The total survival rate and total tumor-free survival rate 3 years after therapy were higher in those patients received surgical resection than in those received radiofrequency ablation in both single pure group and multiple combined group（P〈0.05）. The incidence of major complications resulted in radiofrequency ablation was obviously lower than that in surgical treatment（P〈0.05）. Radiofrequency ablation therapy had the advantages of short hospitalization time, rapid recovery, etc. Conclusions Surgery gives better result in the treatment of primary liver cancer meeting Milan criteria, especially its long-term effect is better than that of radiofrequency ablation.
